Quick answer

Midi Health and Alloy are the best online weight loss programs for most women in menopause, because the same clinicians can prescribe a GLP-1 and menopause hormone therapy. On their own pages in October 2026, compounded medication started at $164 per 4-week supply plus visits at Midi, and $70 a month plus a $99 membership at Alloy.

Key takeaways

  • Midi Health and Alloy are the best fit for most women in menopause, because the same clinicians can prescribe your GLP-1 and your hormone therapy and adjust both.
  • Ro suits you if you want brand-name medication at manufacturer cash prices, Noom suits you if you want one all-in price with coaching, and Henry Meds suits you if you want compounded or non-injectable options with hormone therapy as a separate program.
  • Women who were already on hormone therapy lost more weight on semaglutide and tirzepatide than women who were not, so a program that prescribes both lets one clinical team look at the two together.
  • Most programs charge a membership or visit fee plus medication, and on the programs' own pages in October 2026, brand-name Zepbound started at $299 a month at Alloy and Ro plus a membership, while Noom bundles care and medication into one price.
  • Before you sign up, check how often someone adjusts your dose, what follow-up you get, the real monthly total and whether the program can also prescribe estrogen.

Midi Health and Alloy are the best online weight loss programs for most women in menopause, because both pair GLP-1 prescriptions with menopause hormone care, and Ro, Noom and Henry Meds also prescribe weight-loss medication. On the programs' own pages, checked October 2026, compounded medication starts at $164 per 4-week supply at Midi, and brand-name Zepbound starts at $299 a month at Alloy and Ro, plus a membership. The rest depends on whether you choose brand-name or compounded medication and whether you also want hormone therapy.

What is the best online weight loss program for menopause?

The best program is the one where the same clinicians can prescribe your GLP-1 and your menopause hormone therapy, and adjust both. Midi and Alloy do that. The other three suit narrower needs.

Midi is the best fit if you have insurance and want a women's health clinician. Its visits are covered by most major insurance, it is available in all 50 states, and its care plans can include GLP-1s, lab work, hormone prescriptions and non-hormone prescriptions.

Alloy is the best fit if you want a doctor who specializes in menopause and unlimited messaging for a flat monthly fee. Its weight care is built by menopause-specialized doctors, who adjust your dose, and delivery is free.

Ro is the best fit if you want brand-name medication at manufacturer cash prices. Noom is the best fit if you want one all-in monthly price with coaching. Henry Meds is the best fit if you want compounded or non-injectable options and are willing to pay for hormone therapy as a separate program.

Here is how we judged them. We read each program's own pricing page in October 2026 and kept only programs that show weight-loss medication. We compared what each prescribes, whether it offers menopause hormone therapy, the visit type, insurance and the monthly price. For which medicine to ask for, see our guide to the best weight loss medications for women in menopause. For the wider list of providers, see our guide to the best menopause telehealth providers.

Which programs also prescribe HRT?

Midi and Alloy offer hormone therapy as part of their weight care. Henry Meds offers it as a separate program. Ro's and Noom's weight pages do not show hormone therapy at all.

Henry Meds lists a Women's HRT program apart from its GLP-1 program. It covers estradiol as tablets, patches or Bi-Est cream, with micronized progesterone as tablets or cream.

Alloy puts the two together on one page. Its weight page is titled "Weight Loss HRT - Hormone Therapy", and its FAQ answers the question "Can I take GLP-1 medication with Hormone Replacement Therapy (HRT)?"

Pairing the two matters because of what happened to women already on hormone therapy. In a Mayo Clinic records study of 106 postmenopausal women taking semaglutide, the 16 women on hormone therapy lost 16% of their body weight at 12 months, compared with 12% for the 90 women not on it. The gap held after the researchers adjusted for other differences between the groups. The study shows a link, not proof that hormone therapy caused the extra loss.

The same pattern showed up with tirzepatide. In a records study of 120 matched postmenopausal women followed for a median of 18 months, the women on hormone therapy (oral or skin-applied estrogen, with or without progesterone) lost a median of 17% of their body weight, compared with 14% for women not on it. 45% of the women on hormone therapy lost at least 20%, compared with 18% of the others.

Hormone therapy is a separate decision, and whether it suits you depends on your own health. A program that can prescribe both lets one clinical team look at the two together.

How much does an online menopause weight loss program cost?

Most programs charge a membership or visit fee and then charge for medication. Noom is the exception and bundles the two into one price. On the programs' own pages, checked October 2026, compounded medication starts at $164 per 4-week supply at Midi, plus visits, and at $70 a month plus a $99 membership at Alloy. Brand-name medication with membership at Ro starts at $223 a month (the $74 annual prepaid membership plus the $149 Wegovy pill starting dose). The $149 monthly membership plus the $299 Zepbound KwikPen comes to $448 a month, and higher doses cost more.

Prices checked October 2026 on each program's own page.

Compounded medication keeps the bill down, and two of these programs spell out the catch. Midi's footnote reads: "Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness, or quality before they are made available to patients." Noom's page carries the same warning and notes that metformin is not FDA-approved for weight loss.

Does Noom work for menopause?

Noom Med prescribes GLP-1s with coaching at one all-in price, but its pricing page does not mention menopause or hormone therapy. It offers weight care, not menopause care.

Noom's weight-loss figures come from members' self-reported data. Noom also notes that members who used the app more lost more weight, and says this link "does not imply causation."

Noom suits you if you already get hormone care elsewhere and want structure and a predictable bill. If you want hot flashes, sleep and weight handled by one team, Midi or Alloy fits better.

What should you check before you sign up?

  • How often someone adjusts your dose. A dose increase can bring a rough patch. One r/Zepbound member wrote: "Had a little setback when I upped my dose to 5mg last month and had some pretty rough GI side effects and dehydration." They went back down to 2.5 and said they "have been feeling much better this week."
  • What follow-up you get. Midi offers 15-minute follow-up visits "to track progress and make adjustments." Alloy offers unlimited doctor messaging. Ro includes side effect management and dose adjustments in its membership.
  • The real monthly total. Add membership and medication together using the table above, and check whether compounded options are sold in your state. For how prescribing works online, see how to get a GLP-1 prescription.
  • Whether the program can also prescribe estrogen if your clinician thinks it fits. If it cannot, you will need a second provider for it.
  • Hot or Nothing is another option. Its plans include semaglutide and tirzepatide for weight and bioidentical estrogen for hormones, and your clinical team decides which fits you. See all weight options.

You want the weight to come off and your hormones looked after by the same people. Take the quiz to see whether a Hot or Nothing plan with semaglutide or tirzepatide, plus estrogen if your clinical team finds it fits, matches your goal.

FAQ

Can you take a GLP-1 and HRT at the same time?

Yes, and Midi and Alloy both prescribe GLP-1s and menopause hormone therapy as part of their weight care. In two medical-records studies, postmenopausal women already on hormone therapy lost more weight on semaglutide and tirzepatide than women who were not, though that shows a link, not proof that hormone therapy caused it. Whether hormone therapy suits you depends on your own health, so your clinical team looks at the two together.

Is Midi or Alloy better for menopause weight loss?

Midi is the better fit if you have insurance, and Alloy is the better fit if you want a menopause specialist on call for a flat monthly fee. Midi's visits are covered by most major insurance, with self-pay visits at $250 for the first and $150 for each return, and compounded medication from $164 per 4-week supply with shipping included. Alloy charges $99 a month for unlimited doctor messaging, with compounded semaglutide from $70 a month and Zepbound from $299, but its compounded options are not available in AL, AR, CA, NV, LA, MS or DC. Prices were checked on each program's own page in October 2026.

Does insurance cover online menopause weight loss programs?

Sometimes, depending on the program and your plan. Midi's visits are covered by most major insurance, and its brand-name GLP-1 cost depends on your coverage. Ro checks coverage for Ozempic, Wegovy and Zepbound autoinjector pens, while Noom says its program is not covered but checks coverage for branded medication and notes that many plans are FSA or HSA eligible. Alloy's weight page and Henry Meds' programs page do not state insurance coverage.

How fast do you lose weight on a GLP-1 after menopause?

Postmenopausal women on semaglutide lost about 5% to 7% of their body weight by 3 months and 12% to 16% by 12 months in a Mayo Clinic records study, with the higher numbers in women on hormone therapy. In a records study of 120 postmenopausal women on tirzepatide, followed for a median of 18 months, women lost a median of 14% to 17% of their body weight, again more with hormone therapy. A dose increase can bring a rough patch of stomach side effects, which is why how often someone adjusts your dose matters.
