What is the best over the counter estrogen cream, and do any contain real estrogen?

Quick answer

For vaginal dryness or painful sex, the best over the counter choice is a hormone-free, long-acting vaginal moisturizer, because no over the counter estrogen cream is FDA-approved. Vigority's estriol cream is sold as an untested face cream, and Estrace estradiol vaginal cream needs a prescription. Prescription estrogen does more for severe dryness.

Key takeaways

  • No over the counter estrogen cream is FDA-approved for vaginal dryness: Vigority sells estriol as a face cream, and Estrace estradiol vaginal cream needs a prescription.
  • Wild yam and the other plant ingredients on these labels are not estrogen, and your body cannot turn wild yam into progesterone.
  • Estriol cream reaches your blood within about 2 hours, Vigority's label claims ten times the dose that has been measured, and bleeding after menopause means you stop the cream and call your clinical team.
  • A hormone-free vaginal moisturizer eases dryness about as well as prescription estriol cream within 6 weeks, with fewer side effects.
  • Prescription estrogen suits you if your dryness is severe or a moisturizer has not done enough, and your clinical team checks your health history before prescribing it.

For vaginal dryness or painful sex, no over the counter cream is FDA-approved estrogen: Estrace needs a prescription, Vigority sells estriol as a face cream, and hormone-free vaginal moisturizers have been tested against estrogen cream. We judged each product on four things: what its label lists, whether a named study tested it, whether it has FDA approval or clearance, and the price on the seller's own page in November 2026. If you want real estrogen, our clinical team can check whether prescription bioidentical estrogen fits you.

Do any over the counter estrogen creams have real estrogen in them?

Yes, some do. Vigority Estriol Face Cream on Amazon lists estriol in its ingredients. The listing says each pump delivers 5,000 mcg of "natural bioidentical estrogen," blended with wild yam, kudzu root, soy isoflavones and red clover.

Estriol is a real human estrogen, but no FDA-approved product contains it. A review in the Journal of the American Board of Family Medicine reports this. It also notes that in 2008, FDA warned that estriol may not be compounded without an investigational new drug application.

Vigority sits in Amazon's Beauty and Personal Care section as a face cream "for external use only." The directions say one pump daily on the face, neck, chest, inner thighs or arms. That is a skincare routine, not a dose anyone has checked for you.

The plant ingredients on these labels are not estrogen. Wild yam is a common one. The same review says humans cannot convert yams into progesterone, and it does not recommend over the counter yam cream.

Soy isoflavones have been tested in the vagina. A systematic review of 4 randomized trials of topical isoflavones in postmenopausal women found they helped painful sex and dryness. The reviewers said the trials were too few, too small and too weakly designed for a firm conclusion. None of the trials tested a face cream.

Is estriol cream from Amazon safe to use?

Nobody has shown that it is. Estriol cream gets into your blood, and Vigority's label claims ten times the dose researchers have measured.

In an Australian study of 10 long-term estriol cream users, the women were aged 51 to 83, with an average age of 70. A 0.5 mg vaginal estriol dose peaked in their blood about 2 hours after use. Every woman went above the postmenopausal cut-off of 100 pmol/L. In 9 of the 10, levels fell back to 10 pmol/L or less by 24 hours. The authors said the safety of topical estriol still has to be established.

Vigority's label claims 5,000 mcg per pump, which is ten times that 0.5 mg dose. You put it on your face rather than in the vagina, and no study we read measured what it puts in your blood.

Vigority calls estriol "the gentlest and safest form of estrogen." The review calls it a weaker estrogen that is not necessarily benign, and says its breast cancer safety is unproven. The review also says that estriol given often enough to build up in the blood stimulates breast and uterine tissue, much as estradiol does.

One Vigority buyer wrote in an Amazon review: "after only two weeks of use I had to stop using it because I started spotting, after not having a menstrual cycle for almost a year."

Estrace's FDA label lists undiagnosed abnormal genital bleeding as a reason not to use estrogen. It also says a woman with a uterus who has bleeding that continues or comes back needs tests to rule out cancer, including a sample of the uterine lining when indicated. Bleeding after menopause is a reason to call your clinical team, not to keep using the cream.

What is the best over the counter option for vaginal dryness?

A hormone-free, long-acting vaginal moisturizer is the over the counter option with head-to-head data against estrogen. The North American Menopause Society lists these moisturizers as first-line treatment. Low-dose vaginal estriol or estradiol is recommended when they are not enough.

In a 6-week randomized trial in postmenopausal women, a hormone-free moisturizing cream relieved dryness symptoms about as well as a prescription estriol cream. Symptom scores fell from 6.1 to 1.1 in the 80 women on the moisturizer and from 6.0 to 0.6 in the 71 women on estriol. Fewer women reported side effects on the moisturizer: 33 of 87, against 46 of 85 on estriol. Dr. August Wolff, the company that develops the moisturizer, paid for the study.

Revaree by Bonafide is a hormone-free insert with 5 mg of hyaluronic acid in each one. You use one at bedtime every 2 to 3 nights, and a box of 30 lasts 3 months. Bonafide says Revaree is FDA-cleared as a Class II medical device. It is not compatible with condoms.

When should you get prescription estrogen cream instead?

Consider prescription estrogen when your dryness is severe or a moisturizer has not done enough. Prescription estrogen did more for the vaginal tissue itself.

In the 6-week trial, estriol cream beat the moisturizer on the doctor's exam of vaginal health. It also did more to reduce how much dryness got in the way of daily life. That advantage held only for women whose dryness was severe when they started. For women with mild or moderate dryness, the difference was not significant.

A prescription gets you real estrogen in a checked dose. According to the review, FDA approval requires safety and efficacy data. It also standardizes products for purity and potency, with little variation from batch to batch. FDA-approved bioidentical hormones come as vaginal tablets, creams, gels, patches and rings.

Estrogen is not for everyone. Estrace's label rules it out for women with any of these:

  • A history of breast cancer
  • Blood clots
  • Stroke or heart attack
  • Liver disease
  • Undiagnosed genital bleeding
  • Pregnancy

The label warns that some estrogen may be absorbed into the body. It says to use the lowest dose that controls symptoms. Your clinical team checks all of this before prescribing.

Hot or Nothing is a third option alongside the drugstore and your usual doctor. Our clinical team can prescribe bioidentical estrogen if it fits your goal and your health history. You can compare forms in our guide to vaginal estrogen or see our hormone plans.

FAQ

Can you buy estradiol cream over the counter?

No, estradiol vaginal cream such as Estrace needs a prescription. Its FDA label approves it for moderate to severe vaginal dryness and thinning caused by menopause, and advises the lowest dose that controls symptoms. Our clinical team can check whether prescription bioidentical estrogen fits your goal and your health history.

Is estriol cream safer than estradiol cream?

Nobody has shown that estriol cream is safer. Estriol is a weaker estrogen, but a review in the Journal of the American Board of Family Medicine says it is not necessarily harmless, its breast cancer safety is unproven, and estriol that builds up in the blood stimulates breast and uterine tissue much as estradiol does. No FDA-approved product contains estriol, while Estrace estradiol vaginal cream is FDA-approved.

Is Revaree as good as vaginal estrogen?

No study linked in this article compares Revaree with vaginal estrogen. A different hormone-free moisturizing cream eased dryness about as well as prescription estriol cream over 6 weeks, but the estrogen did more for the vaginal tissue and for women whose dryness was severe. Revaree is a hormone-free hyaluronic acid insert you use every 2 to 3 nights, and it is not compatible with condoms.

Can estrogen cream cause bleeding after menopause?

Yes, it can. One Vigority buyer said she started spotting after two weeks on the estriol face cream, after almost a year without a period. Bleeding after menopause is a reason to stop the cream and call your clinical team, because Estrace's FDA label says bleeding that continues or comes back needs tests to rule out cancer.
