Quick answer

The best shampoos for thinning hair in women contain ketoconazole, which suits a flaky, itchy or oily scalp, or caffeine, which suits a calm scalp that sheds more than usual. No shampoo has matched minoxidil, the approved treatment for female pattern hair loss. Use the shampoo to keep your scalp comfortable and let minoxidil do the regrowing.

Key takeaways

  • No shampoo has matched minoxidil, the approved treatment for female pattern hair loss in women.
  • Ketoconazole left on the scalp helped thinning hair more slowly than minoxidil, showing results at six months instead of four, but it was much easier on the scalp.
  • Ketoconazole shampoo suits a flaky, itchy or oily scalp: Nizoral 1% costs $15.88 for 7 oz at H-E-B, and the 2% version needs a prescription.
  • A plant-and-caffeine shampoo cut how much hair came out over six months for women with thinning hair, but other caffeine shampoos, Nioxin included, may not do the same.
  • If you use minoxidil, wash with a gentle everyday shampoo and add a ketoconazole wash twice a week if your scalp flakes or itches.

The best shampoos for thinning hair in women contain ketoconazole (a good fit if your scalp is flaky, itchy or oily) or caffeine (a good fit if you mainly notice shedding). Both have small studies behind them, and both work best as support. Minoxidil is the approved treatment for female pattern hair loss, so choose your shampoo to keep your scalp comfortable while minoxidil does the regrowing.

Can a shampoo regrow thinning hair?

No shampoo has matched minoxidil. Topical minoxidil is approved for female pattern hair loss. The FDA approved the 2% solution first, at 1 ml twice a day. In 2014, the FDA and Health Canada approved the 5% strength. A 2019 trial comparing ketoconazole and minoxidil in women describes topical minoxidil as the only approved treatment for female pattern hair loss.

Ketoconazole has been tested against minoxidil. In that six-month trial, 40 women with female pattern hair loss used either 2% minoxidil solution or 2% ketoconazole, 1 ml a day. The ketoconazole was a compounded preparation that stayed on the scalp, not a shampoo that rinses off. It helped, but more slowly. Women on minoxidil had improved by month 4, and the ketoconazole group improved only at month 6.

Ketoconazole was easier on the scalp. In the same trial, 10% of women on ketoconazole reported side effects, compared with 55% on minoxidil. Satisfaction did not differ in a statistically significant way between the two groups.

A shampoo sits on your scalp for a few minutes and then rinses away. Treat it as the part of your routine that keeps your scalp clean and comfortable, and let a treatment you leave on do the regrowing.

Which shampoos for thinning hair have studies behind them?

Shampoo type What the research showed Where to get it and price Scalp it fits
Ketoconazole 2% (prescription) In men with pattern hair loss, long-term use of 2% ketoconazole shampoo improved hair density by about as much as 2% minoxidil (study summary) Prescription only in the US. Through Miiskin, a consultation is $59 and refills are $39. Generic 2% shampoo costs $18 for 120 ml at Foothills, the pharmacy Miiskin names Flaky, itchy or oily
Ketoconazole 1% (Nizoral, over the counter) Labeled for dandruff flaking, scaling and itching, not hair loss $15.88 for 7 oz at H-E-B Flaky, itchy or oily
Caffeine shampoo In a six-month trial in women with pattern hair loss, a plant-and-caffeine shampoo reduced hairs pulled out in a pull test by 3.1, compared with 0.5 for a plain shampoo. Women tolerated it well Sold as a cosmetic shampoo Calm scalp, with more shedding than usual
Nioxin System 2 (volumizing, with caffeine, biotin and niacinamide) No trial of this shampoo. The brand promises visible results in 30 days $22.99 for 10.1 oz at Target Any scalp, if you want fuller-looking hair

The ketoconazole shampoo result comes from men, and the authors of that 1998 study of long-term ketoconazole shampoo said its clinical significance would need a larger controlled study before anyone could judge it. The caffeine trial was double-blind. Neither the women nor the researchers knew which shampoo each woman had. It tested one specific formula, so another caffeine shampoo may not give the same result.

That includes Nioxin. Its caffeine content does not mean it will match the trial formula. Many of its Target reviews are marked as collected as part of a promotion. One reviewer wrote that it is "not a direct solution for hair regrowth."

Which shampoo fits my scalp?

If your scalp flakes, itches or gets greasy fast, start with ketoconazole. The Nizoral label covers flaking, scaling and itching. It tells you to use it twice a week, between your regular shampoos.

If your scalp feels calm and you mainly see more hair in the drain or on your brush, a caffeine shampoo fits. In the trial in women, it reduced how much hair came out. Hair loss intensity and hair strength also improved compared with the plain shampoo.

If you want your hair to look fuller tonight, a volumizing shampoo can help with the look. Buy it for that reason and not for regrowth.

If your part is getting wider or your ponytail is thinner, no shampoo has shown it can fix that by itself. That is when minoxidil belongs in your plan. Topical minoxidil stops hair loss from getting worse, regrows hair, or both, in most women. Our clinical team can prescribe minoxidil if it fits your goal.

What shampoo should I use with minoxidil?

Use a gentle everyday shampoo, and add a twice-weekly ketoconazole wash if your scalp flakes or itches. One woman described her experience after eight months on topical minoxidil: "dry scalp/flaking: my doc prescribed Keto shampoo which worked well but my insurance changed so now I use Nizoral which has a strong masculine smell that I actually really like. Helps a lot."

She also had to wash more than once to get the minoxidil out: "topical minoxidil seems to coat the hair pretty thickly so I find myself double and sometimes triple shampooing to get that 'squeaky clean' feeling." If minoxidil leaves your hair coated too, your everyday shampoo does the cleaning, and ketoconazole handles the flakes.

Mild itching or flaking can happen in the first one to two months of minoxidil. If it keeps going or starts later, it can be an allergy to minoxidil itself or to propylene glycol, an ingredient in many minoxidil solutions. In one reported case, a woman allergic to propylene glycol switched to a 5% minoxidil foam without it after her scalp healed. Tell your clinical team if your scalp gets red, scaly or sore.

The Nizoral label says not to use it on broken or inflamed skin. Stop and ask a doctor if a rash appears, or if your scalp gets worse or does not improve in 2 to 4 weeks. If you are pregnant or breastfeeding, ask before using it.

Some women on topical minoxidil grow extra facial hair. The same woman wrote: "Facial hair growth including lashes and brows. I'm okay with it. I now shave my face monthly."

A good shampoo keeps your scalp calm, but minoxidil regrows the hair. Take the quiz so our clinical team can see whether minoxidil fits your goal, and use your shampoo to support it.

FAQ

How often should I use Nizoral for hair loss?

Use Nizoral twice a week, between your regular shampoos, which is what the label directs. The label covers dandruff flaking, scaling and itching, not hair loss, so it fits best when your scalp is flaky, itchy or oily. If you use minoxidil, a gentle everyday shampoo plus a twice-weekly Nizoral wash is a common routine. Stop and ask your clinical team if a rash appears or your scalp does not improve in 2 to 4 weeks.

Is biotin shampoo good for thinning hair?

No biotin shampoo has been shown to regrow thinning hair. Nioxin System 2 contains biotin along with caffeine and niacinamide, but that shampoo has not been tested in a trial. A volumizing shampoo like this can make hair look fuller, so buy it for the look and not for regrowth.

Can the wrong shampoo make hair fall out?

Shampoo is not what causes female pattern hair loss, which is driven by genetics and hormones, but overusing a medicated shampoo can damage your hair. A dermatologist quoted by Miiskin says using too much ketoconazole can dry and irritate the scalp and leave hair brittle and prone to breaking. Alternating it with a gentle shampoo helps. Tell your clinical team if your scalp gets red, scaly or sore.

How long does minoxidil take to work for women?

Women using 2% minoxidil on the scalp showed improvement by month 4 in a six-month trial. Mild itching or flaking can show up in the first one to two months. If it keeps going or starts later, tell your clinical team, since it can be an allergy to minoxidil or to propylene glycol, an ingredient in many minoxidil solutions.
