Quick answer
Epithalon (also spelled epitalon) has not been shown to lower cortisol in people, because nobody has checked cortisol in humans taking it. In older female monkeys, epithalon lowered evening cortisol and left morning cortisol where it was, which brought back the normal pattern of higher cortisol in the morning and lower cortisol at night. FDA says it is unknown what that monkey result means for people.

Key takeaways
- Epithalon has not been shown to lower cortisol in people, and nobody has checked cortisol in humans taking it.
- In older female monkeys, epithalon brought evening cortisol down, left morning cortisol alone and raised evening melatonin, while in young monkeys cortisol dropped both morning and night.
- In women who took epithalon under the tongue every day for 20 days, a sign of melatonin in their urine went up, but their cortisol was never checked.
- FDA says it does not have enough information to know whether epithalon causes harm in people, warns it can clump and carry impurities that may set off an immune reaction, and many websites selling it label it for research use only.
- If stress and bad sleep are wearing you down, tell your clinical team when you wake up, how you feel in the afternoon and where you are in your cycle or menopause, because hormones can be behind broken sleep and anxiety.
Epithalon has not been shown to lower morning cortisol in people. In the one study that measured cortisol, older female monkeys given epitalon (the same peptide, spelled two ways) had about 30% lower evening cortisol and no significant change in morning cortisol, according to FDA's review of the monkey study. That restored the normal pattern of high cortisol in the morning and low cortisol at night. No human study we found measured cortisol, and FDA says the meaning of the monkey result is unknown.
What did epithalon do to cortisol in studies?
The cortisol numbers come from one study of young (6 to 8 years) and older (20 to 26 years) female rhesus monkeys, described in FDA's epitalon briefing. Researchers measured melatonin and cortisol in the blood. Cortisol is the stress hormone your adrenal glands make. It normally runs highest in the morning and lower at night.
Before treatment, the young monkeys had that rhythm, with cortisol at about 1,100 nmol/L in the morning and about 600 at night. The older monkeys had lost it and sat around 900 both morning and night.
| Monkeys | Morning cortisol | Evening cortisol | Evening melatonin |
|---|---|---|---|
| Older (20 to 26 years) | No significant change | Down about 30% | Up more than fourfold |
| Young (6 to 8 years) | Down | Down | No significant change |
In the older monkeys, the researchers described the result as bringing the cortisol rhythm back to normal. The evening level dropped, the morning level held and the day-to-night swing returned. In the young monkeys, cortisol fell at both times. The effect depended on age, and the study does not tell us whether the drop in young animals was good or bad.
Does epithalon lower morning cortisol in women?
Nobody knows yet. We found no human study that measured cortisol after epithalon, so any claim that it lowers your morning cortisol goes past the evidence.
The human study that does exist measured sleep hormones, not cortisol. In a study of 75 women, epitalon taken under the tongue at 0.5 mg a day for 20 days raised a urine marker of melatonin 1.6 times compared with placebo. It also changed the activity of several body-clock genes.
FDA reviewed the monkey cortisol result and wrote that "the functional relevance of the finding that epitalon treatment of older monkeys increased evening serum levels of melatonin and decreased evening serum levels of cortisol is unknown."
If your morning cortisol is what worries you, the data from older animals points the other way. In the older monkeys, morning levels stayed put. The evening level was what changed.
How does epithalon affect the HPA axis and stress?
The HPA axis is the chain from your brain (the hypothalamus and pituitary) to your adrenal glands that controls cortisol when you are stressed. The authors of the monkey study reported that epitalon raised evening melatonin in older animals, "thereby normalising the circadian rhythm of cortisol secretion." They did not test how the monkeys responded to stress.
The stress data comes from rats. In stressed rats, epitalon given in the nose slightly raised a marker of nerve-cell activity in the pineal gland, the small brain gland that makes melatonin. This happened only in the stressed rats. The same marker also rose in parts of the hypothalamus. What that means for how stress feels is not known.
How epitalon works on melatonin is also unclear. FDA says the way it affects melatonin and body-clock rhythms is unknown, and it found no animal study that tested its effect on sleep itself. In one lab study, epitalon did not make isolated rat pineal glands release more melatonin, at any dose or age. Whatever raised melatonin in the monkeys and women may work through some other route.
Is epithalon safe, and can you get it prescribed?
FDA's compounding advisory committee met in July 2026 to discuss whether epitalon should go on the list of substances pharmacies can compound from. The use FDA reviewed was insomnia.
FDA's safety note on compounded epitalon says it "may pose risk for immunogenicity for certain routes of administration due to the potential for aggregation and peptide-related impurities." In plain terms, the peptide can clump and carry impurities, which may set off an immune reaction. FDA adds that it "lacks sufficient information to know whether the drug would cause harm if administered to humans." FDA lists the epitalon nomination as withdrawn and kept this note.
In a Google search, FDA could not find any pharmacy compounding epitalon, though some wellness clinics say they get it from one. Several websites sell it as 10 to 50 mg vials, capsules or an oral spray, and several of them say it is for research use only.

What can help if stress and bad sleep are wearing you down?
Some people who land on epithalon are exhausted and sleeping badly, and they get handed a long list of peptides. One poster in r/Peptides wrote: "I am sleepy 24/7 and I can't seem to sleep through the night most nights." The poster had been told to try SS-31 with MOTS-c, tesamorelin with ipamorelin, sermorelin, epitalon and DSIP. That is five options for one problem nobody had looked at yet.
For women in perimenopause or menopause, broken sleep and anxiety can be hormonal. In r/Menopause, the top comment on one thread said: "Yes, the estrogen patch and oral progesterone eliminated my insomnia and greatly reduced (although did not eliminate) both anxiety and panic attacks." That was her experience, and it came from treating her hormones, not her cortisol.
Before you chase a cortisol fix, tell your clinical team when you wake up, how you feel in the afternoon and where you are in your cycle or menopause. Waking at 3am, crashing at 3pm and feeling wired at bedtime can have different causes, and the pattern points to the treatment. If hormones are part of it, our clinical team can prescribe bioidentical estrogen when it fits your goal.
If stress, 3am wake-ups or afternoon crashes sent you looking for epithalon, take the quiz so our clinical team can see what is behind your sleep and energy and suggest a plan that fits.

Does epitalon raise melatonin in humans?
In one group of women, epitalon taken under the tongue at 0.5 mg a day for 20 days raised a urine marker of melatonin about 1.6 times compared with placebo. It also changed the activity of several body-clock genes. How epitalon does this is unknown, and in a lab test it did not make rat pineal glands release more melatonin. FDA found no animal study that tested its effect on sleep itself.
Is epitalon FDA approved?
No FDA-approved epitalon product is available, according to the nomination sent to FDA. FDA's compounding advisory committee met in July 2026 to discuss whether pharmacies could compound epitalon, and the use FDA reviewed was insomnia. FDA could not find any pharmacy compounding it, and several websites that sell it say it is for research use only.
What are the side effects of epitalon?
Nobody knows the side effects of epitalon in people well, because FDA says it lacks enough information to know whether it would cause harm in humans. FDA's main worry is that compounded epitalon can clump and carry impurities, which may set off an immune reaction for some ways of taking it. Epithalon is not part of Hot or Nothing's plans.
Does epitalon lengthen telomeres?
Epitalon has only been shown to lengthen telomeres in human cells grown in a lab, not in people. Telomeres are the protective caps on the ends of your chromosomes that shorten with age. In those lab cells, epitalon switched on the enzyme that builds telomeres and lengthened them by about a third on average. Whether that happens in your body is not known.
