A 2025 review in Current Opinion in Obstetrics and Gynecology called GLP-1s consistently the most effective medicines for weight loss and a valuable tool for peri and postmenopausal women, while also noting very little data specific to this group review.
The most detailed hormone therapy comparison followed 106 postmenopausal women on semaglutide. The 16 women also using hormone therapy, which means estrogen with or without progesterone for menopause symptoms, lost more weight at every check. At 12 months they had lost 16% of body weight compared with 12% without hormone therapy. Both groups improved blood sugar, blood pressure and lipids Mayo cohort. That was a small, look-back study, so it shows a link, not a promise that hormone therapy raises loss for everyone.
A separate analysis of 2,542 women from the SURMOUNT-1, SURMOUNT-3 and SURMOUNT-4 tirzepatide trials followed women up to 88 weeks. Perimenopausal and postmenopausal women lost weight comparably to premenopausal women when they used tirzepatide with lifestyle changes menopause analysis.
How long before you see meaningful loss?
Menopause studies tell you whether loss happens. General trials tell you how long it can take. In a post hoc analysis of 1,545 people on tirzepatide in SURMOUNT-1, 278 people or 18% had lost less than 5% at week 12. By week 24, 70% of those slower starters had reached 5% or more loss. By week 72, 90% had reached 5% or more. The average time to reach 5% for that slower group was about 25 weeks late-responder analysis.
That pattern matters in perimenopause because early weeks can feel discouraging. A smaller number on the scale at 12 weeks did not mean later months failed for most people in that analysis.
