Why didn't HRT bring my desire back?

You can feel cooler at night and more comfortable day to day and still feel no pull toward sex. That split is common because estrogen eases flashes and vaginal dryness but desire also tracks testosterone availability, mood, sleep and relationship factors.

Oral estrogen changes how much testosterone floats free. SHBG, a protein made in the liver that holds onto sex hormones, rises with oral estrogen, so less testosterone stays free to act. In a JCEM study, oral estrogen users had SHBG of 182.0 versus 82.9 nmol/L in nonusers and free testosterone of 3.7 versus 7.9 pmol/L JCEM 2000 study.

Route mattered for that lab change. In a comparison of 443 postmenopausal women, transdermal estradiol left SHBG close to levels in women not on HRT, while all oral estrogen groups showed higher SHBG route comparison. That comparison measured blood levels. Researchers did not track change in desire in that comparison, so it does not show that switching routes restores desire.

Can adding testosterone help when HRT hasn't?

In a 52-week trial in 814 postmenopausal women with low desire causing distress who were not taking estrogen, a patch that delivered 300 micrograms of testosterone per day led to a modest but meaningful gain in sexual function compared with placebo NEJM trial.

In a UK specialist clinic, 510 women already on HRT with persistent low libido used testosterone cream or gel for 4 months. Fifty-two percent reported better libido, and the average libido symptom score fell 33 percent UK cohort. That clinic review looked back at records without a placebo group, so time, attention and expectation could play a part.

The VA March 2025 summary notes there are no FDA-approved testosterone products for women in the U.S., and FDA declined a patch for women in 2004 because long-term safety data were lacking VA summary. Research doses were about one tenth of a male dose. Researchers aimed for the premenopausal female range. One member of r/Menopause about 3 years ago described a year of estrogen and progestogen HRT that helped other symptoms, wrote desire was “It was basically non-existent” then after starting transdermal gel wrote “My libido is sky-high after less than a month on it” r/Menopause report.

Another member of r/Menopause wrote “The HRT helped with the meno symptoms” yet “for the last year or two its nothing” and called the loss upsetting r/Menopause report.

A third member of r/Menopause wrote testosterone gel “increased my energy overall and interest” and described going from “after two years of basically no sex” to sex once a week or twice a month feeling good enough r/Menopause report.

The pattern is HRT eased menopause symptoms but left desire at zero for a year or more, and some women describe energy and interest returning within weeks of added gel, while others describe ongoing absence and distress. No prices were given in these reports. These stories describe personal experience and do not prove how well or safely testosterone works. Trials carry that weight.
