What Results Can Women Expect from Tesamorelin?

What results do women actually get on tesamorelin?

Quick answer

Tesamorelin can shrink some of the deep belly fat packed around the organs and take a little off the waist, but it does not shrink the soft fat you can pinch or lower the number on the scale. Those results come from adults with HIV and extra belly fat, most of them men, who injected tesamorelin every day for about six months. The deep belly fat came back when people stopped tesamorelin.

Key takeaways

  • Daily tesamorelin injections shrank some of the deep belly fat around the organs in adults with HIV and extra belly fat, most of them men, so there is little data in women.
  • Losing deep belly fat can flatten your stomach somewhat, but tesamorelin leaves the soft fat you can pinch, barely moves the scale and can even push it up at first because it makes you hold water.
  • The deep belly fat comes back within about six months of stopping, so plan on tesamorelin as an ongoing daily injection with a monthly cost.
  • The most common side effects are redness, itching or bruising where you inject, muscle aches, swollen hands or feet and tingling, and tesamorelin can push blood sugar into the diabetes range, so blood sugar gets checked before and during treatment.
  • Compounded tesamorelin sold online is not the brand-name drug Egrifta, and Zepym lists it from $179 a month at a starting dose of 0.6 mg five days a week, well below the daily dose behind the results.

The tesamorelin results with real numbers come from trials in adults with HIV and extra belly fat, and in the largest one, most people were men. In those trials, a daily injection for 26 weeks cut deep belly fat (the fat packed around the organs) by about 14 to 18 percent and took a little off the waist. Body weight stayed about the same. The fat came back in people who stopped.

If you want a flatter stomach or a lower number on the scale, here is which result tesamorelin has data for.

How much belly fat does tesamorelin get rid of?

In the two main trials, tesamorelin cut deep belly fat by 18 percent and 14 percent over 26 weeks. It did not shrink the soft fat you can pinch.

Tesamorelin is a man-made version of the hormone that tells your pituitary gland to release your own growth hormone (review of tesamorelin in HIV-associated lipodystrophy). It does not give you growth hormone directly.

People in the two main trials injected 2 mg every day for 26 weeks. Their deep belly fat, also called visceral fat, dropped 18 percent in one trial and 14 percent in the other. People on placebo went up 2 percent in the first trial and down 2 percent in the second (Egrifta prescribing information).

These trials enrolled adults with HIV and extra belly fat. In the largest one, 86 percent of the 412 people were men (Zepym's tesamorelin page), so there are few women in this data.

The drop was in deep belly fat. Tesamorelin did not change the fat under the skin of the belly by a clinically meaningful amount (tesamorelin review).

Here is what that means in the mirror. Deep belly fat can push your stomach out from the inside, so losing some of it can flatten your stomach somewhat. The soft layer you can pinch stays.

Does tesamorelin make you lose weight?

No. The US label lists "Not indicated for weight loss management" under its limitations of use (Egrifta WR label on DailyMed). In the trials, the waist got a little smaller, but the scale barely moved.

Most rows below come from a meta-analysis that pooled five randomized trials in people with HIV-related belly fat gain.

What was measured Tesamorelin compared with placebo Source
Waist 1.61 cm smaller Meta-analysis of five trials
Trunk fat 1.18 kg less Meta-analysis of five trials
Liver fat 4.28 percentage points lower Meta-analysis of five trials
Lean mass 1.42 kg more Meta-analysis of five trials
Fat under the skin of the belly No significant drop Meta-analysis of five trials
BMI No significant drop Meta-analysis of five trials
Body weight at 26 weeks Study 1: -0.4 kg vs 0.0 kg on placebo. Study 2: +0.5 kg vs +0.3 kg on placebo Egrifta prescribing information

The scale can also go up at first, because tesamorelin can make you hold water. The label says fluid retention can cause swelling, joint pain and carpal tunnel syndrome, and that these either pass or go away when treatment stops (Egrifta WR label).

One man on r/Peptides was taking retatrutide, testosterone and a nightly tesamorelin and ipamorelin blend. He wrote: "Ive gone from 260 to 272 in 6 weeks." A top reply said: "Tesa / IPA Does cause some water weight retention." He was on several drugs at once, so his gain does not show what tesamorelin alone does.

What happens when you stop tesamorelin?

The deep belly fat comes back. In the trials, people who switched to placebo after 26 weeks regained it over the next six months. People who kept injecting held on to their results through week 52 (tesamorelin review).

The label gives the numbers from scans. Between weeks 26 and 52, deep belly fat went up by 25 and 24 square centimeters in the groups switched to placebo. In the groups that stayed on tesamorelin, it changed by +3 and -11 square centimeters (Egrifta WR label).

So plan on tesamorelin as an ongoing daily injection with a monthly cost. A short course will not reset your waist for good.

What are the side effects of tesamorelin for women?

The most common side effects are reactions where you inject, along with muscle aches, swelling and tingling. It can also raise blood sugar. All figures below come from the Egrifta WR label.

In the first 26 weeks, 25 percent of people on tesamorelin had injection site reactions such as redness, itching, pain or bruising. On placebo, 14 percent did. The label advises rotating where you inject on your belly.

More people on tesamorelin than on placebo had muscle aches (6 vs 2 percent), swollen hands or feet (6 vs 2 percent) and tingling (5 vs 2 percent).

In the trials, 5 percent of people on tesamorelin developed blood sugar in the diabetes range (an HbA1c of 6.5 percent or higher). On placebo, 1 percent did. The label calls for blood sugar checks before and during treatment, plus regular checks of IGF-1, a marker of growth hormone activity.

Do not use tesamorelin if you are pregnant, and stop it if you become pregnant. The patient information also says not to use it if you have active cancer or a problem with your pituitary gland.

How much does tesamorelin cost online, and is it the same drug?

Online clinics sell compounded tesamorelin, which is made to order by a pharmacy. It is not the brand-name product from the trials, and some clinics start at a much lower dose.

Zepym lists tesamorelin from $179 a month, with a starting dose of 0.6 mg injected five days a week. Zepym's own page states that compounded tesamorelin is not the brand-name product that was studied (Zepym's tesamorelin page).

The trial numbers in this article come from the brand-name drug, Egrifta, at 2 mg every day. The newer version, Egrifta WR, is dosed at 1.28 mg once a day, which the label shows is comparable in how much of the drug reaches your body (Egrifta prescribing information). A starting dose of 0.6 mg five days a week is well below the dose behind these results.

The price pushes some people toward other options that work through growth hormone. One r/Peptides user asked: "is it better to stick with sermorelin for price and dosage?" The research here does not compare the two drugs' results. Our clinical team can prescribe sermorelin if it fits your goal.

Tesamorelin's data shows a smaller deep belly and a scale that stays put, so start from what you want to change. Take the quiz, and your clinical team will look at your goal and match you with a plan from our treatments, including GLP-1s such as semaglutide and tirzepatide, and sermorelin.

FAQ

Is tesamorelin FDA approved for women without HIV?

No. The FDA label approves tesamorelin only to reduce excess belly fat in adults with HIV who have HIV-related fat gain around the middle. The label also says tesamorelin is not meant for weight loss. Compounded tesamorelin sold by online clinics is not FDA approved for any use.

How long does tesamorelin take to work on belly fat?

The drop in deep belly fat was measured after 26 weeks of daily injections, which is about six months. People who kept injecting held their results through a full year. People who stopped after six months regained the deep belly fat over the next six months.

Is compounded tesamorelin the same as Egrifta?

No. Compounded tesamorelin is made to order by a pharmacy and is not the brand-name Egrifta used in the trials, and Zepym's own tesamorelin page says so. The FDA has not reviewed compounded products for safety, effectiveness or quality. Some clinics also start at a much lower dose, such as Zepym's 0.6 mg five days a week, compared with 2 mg every day of Egrifta in the trials.

Does tesamorelin cause water retention?

Yes, tesamorelin can make your body hold water. The label says this can cause swelling, joint pain and carpal tunnel syndrome (pain or numbness in the hand and wrist), and that these either pass or go away when treatment stops. Swollen hands or feet showed up in 6 percent of people on tesamorelin compared with 2 percent on placebo. On r/Peptides, users say tesamorelin can add water weight, though the man who posted his gain was taking several drugs at once.
