Insurance usually says yes to a GLP-1, a drug that mimics a gut hormone to curb appetite, for diabetes or heart disease and no to the same drug for weight loss alone. The use written on the prescription triggers different rules. Medicare cannot pay for weight loss drugs under federal law, and most work and Marketplace plans leave weight loss out because list prices average about $1,000 a month (KFF Tracking Poll Nov 2025).
Why doesn't Medicare cover weight loss alone?
Federal law prohibits Medicare from covering drugs when prescribed specifically for weight loss. That rule helps explain why only 9% of adults 65 and older report current GLP-1 use (KFF Tracking Poll Nov 2025).
You can still get coverage when your doctor prescribes the drug for diabetes or heart disease through Medicare Part D, the part that pays for prescriptions. Wegovy earned added approval to lower risk of heart attack, stroke and death in adults with heart disease plus overweight or obesity, which opened a Part D pathway for an estimated 3.6 million beneficiaries based on 2020 data (United Medicare Advisors June 2026).
Why do employer and Marketplace plans say no?
Large employers are pulling back as costs rise. A Business Group on Health survey found 60% covered GLP-1s for weight loss in 2026, down from 72% in 2025 (KFF Health News Sept 2026). A 2026 survey by the International Foundation of Employee Benefit Plans found 60% of employers cover GLP-1s for diabetes only and 36% cover both diabetes and weight loss. Among employers that do not cover weight loss use, 83% exclude it with a carve out that writes weight loss drugs out of the drug benefit (IFEBP 2026 survey). In that survey, GLP-1s grew from 6.9% of annual claims in 2023 to 11.4% in 2026.
Marketplace plans draw the same line. A 2024 KFF analysis found most Marketplace plans covered GLP-1s for diabetes, but only 1% covered Saxenda or Wegovy for weight loss and none covered Zepbound for weight loss (healthinsurance.org summary). These numbers describe surveyed plans in those years, so your own plan document still decides whether weight loss use is a covered benefit.
What will you pay if weight loss is not covered?
Without coverage you pay list price. KFF put the average list price at about $1,000 per month in November 2025. United Medicare Advisors listed Wegovy at $1,300 per month in June 2026 and estimated 25 to 33% coinsurance under Part D would leave $325 to $430 per month out of pocket (United Medicare Advisors June 2026).
The Medicare GLP-1 Bridge lowers that bill for a narrow group. From July 1, 2026 through December 31, 2027, eligible Part D members pay a flat $50 per month for Foundayo tablets, Wegovy injection or tablet, or Zepbound KwikPen when prescribed for weight management (CMS Bridge overview). You qualify with Part D coverage plus a body mass index (a weight to height ratio) of 35 or higher, or 30 or higher with certain related conditions. The $50 payment sits outside the Part D benefit and does not count toward your deductible or yearly out of pocket limit.
