Quick answer
For pitted acne scars, the main treatments are in-office procedures such as RF microneedling, lasers, chemical peels, subcision and fillers, often combined, with prescription retinoids as the at-home route. The right one depends on your scar depth, budget, downtime and whether you still break out, which a clinician judges by examining your skin.

Key takeaways
- Adapalene 0.3% gel has the most direct at-home scar data here, from an early study in adults with moderate-to-severe pitted scars. Tretinoin, the retinoid in Hot or Nothing's skin plan, is in the same family but was not studied for scars in these sources.
- RF microneedling runs $800 to $2,500 a session in one guide's estimate, and most patients need about three sessions, so a full course comes to roughly $2,400 to $7,500.
- If you still get breakouts, your plan should treat the acne as well as the scars, because new acne can leave new scars.
This guide covers pitted acne scars (the dented kind, which doctors call atrophic), not raised scars or the flat red and dark marks acne can leave.
What are the main treatments for pitted acne scars?
A network meta-analysis by An and colleagues compares the procedures doctors use for moderate-to-severe pitted scars across randomized trials published from 2010 to August 2025. A network meta-analysis pools many trials so treatments can be compared against each other. This review compared scar scores, the share of patients who improved significantly, satisfaction and pain. Its list of options is lasers, microneedling and RF microneedling, chemical peels, subcision, fillers and combinations of these. This guide does not report a ranking from that review, and the sources used here do not establish one winner for every woman. The descriptions below are in no particular order.
- Lasers remove or heat thin columns of skin so it rebuilds.
- Microneedling makes tiny punctures that prompt the skin to repair itself.
- RF microneedling adds heat by sending radiofrequency energy through the needles into the deeper layers of skin.
- Chemical peels use acids to remove the top layers of skin.
- Subcision uses a needle under the scar to cut the fibers pulling it down.
- Fillers lift a dented scar from underneath.
The sources carry different weight. The procedure review pools randomized trials, the strongest kind of comparison among them. The retinoid evidence below comes from a smaller early study and an expert commentary.
Prescription retinoids work from home. In a 2018 study of adapalene 0.3% gel, adults aged 18 to 50 with moderate-to-severe pitted scars from past acne used the gel for 24 weeks. The authors tested it because scar procedures "may not be suitable or affordable to all patients," and they titled their report "Adapalene 0.3% Gel Shows Efficacy for the Treatment of Atrophic Acne Scars." It was an early study at a single center, and every participant knew they were using the gel.
Tretinoin, the retinoid in Hot or Nothing's skin plan, belongs to the same family. A 2025 commentary in Dermatology and Therapy says adapalene, tretinoin and tazarotene "have demonstrated efficacy in reducing acne and acne sequelae in several studies." Acne sequelae are the marks acne leaves behind, including pitted scars, lingering redness and dark spots. The sources here do not include a scar study of tretinoin itself. Belonging to the same family does not mean tretinoin will give the same scar results adapalene gave.
What does RF microneedling cost, and how soon do you see results?
Medical Spa Locator's guide, published in June 2026, updated in September and reviewed by a physician, lists $800 to $2,500 per session. The guide estimates that most patients need three sessions, which would put a full course at roughly $2,400 to $7,500. It also estimates that results peak at three to six months and last one to two years. These are one industry guide's estimates, not typical outcomes measured in clinical trials.
AestheticMedGuide's May 2026 guide gives a wider estimate of $500 to $4,500 per session and says price depends on the device, the area treated and the provider.
Both figures come from consumer guides, and quotes from clinics near you may differ.
A prescription retinoid takes a different kind of effort. In the adapalene study, participants used the gel daily over a 24-week study period. You apply a retinoid at home and skip the clinic appointments.
How do you choose the treatment that fits you?
Four questions narrow the options.
How deep are your scars? The procedure review and the adapalene study both focused on moderate-to-severe pitted scars, so the adapalene research was not limited to faint marks. A clinician who examines your scars can tell you which route has a realistic chance of improving them.
What's your budget? By the guide estimates above, a course of RF microneedling costs thousands of dollars. A retinoid is a prescription you use at home.
How much recovery time can you take? Procedures mean booked sessions, and the sources here do not give recovery times for each treatment. Ask each provider what your skin will look like over the days after a session. A retinoid fits into your usual skincare routine.
Are you still breaking out? New acne can leave new scars. The trifarotene trial, titled "Trifarotene Reduces Risk for Atrophic Acne Scars," enrolled 121 people aged 17 to 34 who had active acne and existing scars. It tells you about scarring during active acne in younger people. It tells you little about a 45-year-old with old scars and clear skin. If you still get breakouts, mention them to your clinician so your plan treats the acne as well as the scars.
If you choose a procedure, ask each provider which device and settings they use, how many sessions they expect you to need and what the full course will cost. Those answers make quotes easier to compare.
If the at-home route appeals, take the quiz. It starts you on Hot or Nothing's doctor-led care, where a clinician assesses your scar type, your skin and your history, including any pregnancy plans, and decides whether tretinoin belongs in your plan.

Which acne scar treatment is best for adult women?
The sources here do not name one treatment as best for every woman. The main options for pitted scars are RF microneedling, lasers, chemical peels, subcision and fillers, often used in combination, plus prescription retinoids at home. A clinician who examines your scars can tell you which has a realistic chance of improving them.
Can tretinoin treat pitted acne scars?
These sources include no scar study of tretinoin itself, so they cannot show how much it improves pitted scars. Its close relative, adapalene 0.3% gel, was tested on moderate-to-severe pitted scars in an early study. A 2025 commentary says adapalene, tretinoin and tazarotene have reduced acne and the marks it leaves in several studies. A clinician can decide whether tretinoin belongs in your plan.
How much does RF microneedling for acne scars cost?
One med spa directory estimates $800 to $2,500 per session, with most patients needing three sessions, for roughly $2,400 to $7,500 in total. Another consumer guide gives $500 to $4,500 per session, depending on the device, the area treated and the provider. Quotes from clinics near you may differ.
How long does RF microneedling take to show results?
One industry guide estimates that results peak at three to six months and last one to two years. These figures are the guide's estimates rather than outcomes measured in clinical trials.
