Most of Semax's clinical research is in people with poor blood flow to the brain, not in healthy women with brain fog. A Russian study followed 187 patients with this problem. Those treated with Semax improved and had a lower risk of stroke and mini-strokes. The authors reported few side effects, and older patients tolerated it well too (Semax in poor brain blood flow).
The studies say little about how Semax feels day to day. One r/Nootropics user, about two weeks in, wrote: "It doesn't make me feel wired or high, it's allowed me to regain focus and shake to the feeling of not caring" (Semax and anhedonia thread). That is one person's experience after two weeks, not proof.

Is Semax safe, and can you get it legally in the US?
FDA says it doesn't have enough information to know whether compounded Semax would cause harm in people. It also says Semax may trigger an immune reaction when taken certain ways, because the peptide can clump and carry impurities.
FDA says much the same about Selank. It lacks important safety information on Selank in people, and the same immune-reaction risk applies. People asked FDA to add Semax and Selank to its compounding list, and those requests were later withdrawn. That withdrawal is not a finding that the peptides are safe.

Researchers link the drop in estrogen during perimenopause to changes in the brain. The only randomized trial in perimenopausal and menopausal women in this research tested creatine hydrochloride, which is not a peptide. Over eight weeks, a medium dose improved reaction time by 6.6%, compared with 1.2% on placebo. It also raised creatine in the front of the brain by 16.4%, compared with 0.9% on placebo.
Women on the medium dose also reported less fatigue, fewer mood swings and less trouble concentrating than when they started. Those changes were measured against each woman's own starting point, not against placebo. Every version was well tolerated, with no severe side effects.
