Quick answer
Nobody knows yet whether Semax clears perimenopause brain fog, because Semax hasn't been tried in perimenopausal women. In healthy men, one nasal dose of Semax sharpened attention and memory and kept them working well for 20 to 24 hours, and stroke patients on Semax got back daily function faster. Semax is available by prescription in the US but is not FDA approved, so if you try it, set a short, tracked test period with a prescriber and don't count on a sure fix.

Key takeaways
- One nasal dose of Semax sharpened attention and memory in healthy men and kept workers performing well for 20 to 24 hours.
- Semax likely works by raising BDNF, a growth signal that helps brain cells connect, but that link so far comes from rats, and most of the evidence on Semax is small, more than 25 years old and from the team that created it.
- Semax hasn't been tried in perimenopausal women or in ADHD, so results in healthy men and stroke patients may not carry over to fog from poor sleep or shifting hormones.
- Semax is sold by prescription through compounding pharmacies but is not FDA approved, and at Rx Peps Direct an order costs $179 ($125 for a 15 mL spray, a $39 medical review and $15 overnight shipping) in 28 states and Washington, DC.
- If you try Semax, pick one symptom to change, track your sleep and cycle, and set an end date with your prescriber, and if your fog came with perimenopause, our clinical team can look at your sleep, energy and hormones through the quiz.
Semax is a nasal spray peptide. Small, older studies found it improved attention, memory and work stamina, mostly in healthy men and in stroke patients. We found no study of Semax in perimenopausal women or in ADHD, so whether it clears your brain fog is still an open question. If you want to try it, treat it as a short, tracked trial with a prescriber, not a sure fix.
What does Semax do for focus and mental stamina?
In the human studies, Semax sharpened attention and memory, and people kept working well for most of a day after one dose. In a 1997 review of 15 years of Semax research, the scientists who developed it reported that it significantly improved memory and attention in healthy men working under extreme conditions. The effects lasted 20 to 24 hours after a nasal dose.
A 1996 study in human volunteers found the same staying power. One nasal dose of 0.25 to 1.0 mg improved how well workers did their jobs for 20 to 24 hours. It also changed their brain waves the way typical focus drugs do.
The likely reason is BDNF, a growth signal that helps brain cells form connections. In a rat study, one nasal dose raised BDNF in the hippocampus, the brain's memory center, by up to 1.4 times. That is an animal result. It shows a plausible mechanism, not proof that Semax works the same way in your brain.
The research team that developed Semax wrote most of this work. The studies were small, and much of it is more than 25 years old. Read the results as promising early signals.
Has Semax been tested in women with perimenopause brain fog or ADHD?
No. The studies looked at stressed workers, healthy men and stroke patients.
| Who was studied | What was measured | What changed |
|---|---|---|
| Healthy volunteers and workers in demanding jobs (1996 study) | Work efficiency and brain waves | Better performance for 20 to 24 hours after a nasal dose |
| Healthy men under extreme work conditions (1997 review) | Memory and attention | Both improved |
| 110 people recovering from stroke, 67 of them women, average age 58 (stroke study) | Blood BDNF and daily function | Semax, given at 6,000 mcg a day in two 10-day courses, raised BDNF and sped up recovery of daily function |
None of these groups were perimenopausal women with brain fog, and we found no Semax study in ADHD. Results from stressed men and stroke patients may not carry over to fog driven by sleep loss or shifting hormones.
What does Semax feel like, according to people who use it?
One user described a clearer head without a stimulant buzz. Semax comes up constantly in online talk about focus and energy. In 80 recent Reddit threads about sleep, energy and focus, 58 mentioned Semax or its sister peptide Selank.
In one r/Nootropics thread about anhedonia (anhedonia means losing interest and pleasure in things), the poster wrote after about two weeks on Semax: "It doesn't make me feel wired or high, it's allowed me to regain focus and shake to the feeling of not caring."
The top reply in that thread pushed back: "Be wary of using more drugs to stop drugs. Self medication is self medication". One person's post tells you how Semax can feel to use. It doesn't show what it will do for you.
Is Semax safe and legal to get in the US?
You can get Semax by prescription in the US today, but it is not FDA approved. FDA says it has too little safety information to know whether Semax would cause harm in people.
FDA has also said compounded Semax may carry a risk of immunogenicity (your immune system reacting to the peptide) for some routes, because the peptide can clump and carry impurities.
FDA's pharmacy compounding advisory committee has voted in favor of adding Semax to the list of substances pharmacies may compound, even though FDA staff recommended against it. The vote is advice only. FDA has not approved Semax or added it to that list, and FDA still makes the final decision.
Telehealth sites already sell Semax by prescription from compounding pharmacies, which make the medicine to order. Its status could change while you are using it.
How much does Semax nasal spray cost, and how should I try it?
- Price. Rx Peps Direct charges $125 for a 15 mL spray (250 mcg per spray), plus a $39 medical review on each order and $15 overnight shipping. That comes to $179 an order. It prescribes in 28 states and Washington, DC. The bottle must be used within 30 days of being made, so plan on one bottle a month.
- Pick one target. Before you start, write down the one thing you want to change. Examples: the 3 pm crash, losing words mid-sentence, rereading the same email. You'll know whether it's working.
- Track sleep and your cycle too. Perimenopause fog can rise and fall with both, and you want to know what's moving it.
- Set an end date. Agree on a set trial length with your prescriber, and tell them every medication and supplement you take. FDA says it has little safety information on Semax.
- Get your hormones looked at as well. Hot or Nothing does not offer Semax. If your fog came with perimenopause, our clinical team can look at your sleep, energy and hormones and tell you which of our treatments fit, including bioidentical estrogen and NAD+.
If perimenopause has turned your afternoons into fog, take the quiz. Our clinical team reviews your sleep, energy and hormone symptoms and builds a plan from treatments with clearer answers for women like you.

How long does Semax take to work?
In healthy men, the effect of one nasal dose of Semax on attention, memory and work performance lasted 20 to 24 hours. The research doesn't pin down how fast it starts, and no one has tested how long it takes to lift perimenopause brain fog. One Reddit user said Semax helped them get their focus back after about two weeks of use. Agree on a set trial length with your prescriber so you know when to judge whether it's working.
Does Semax keep you up at night?
The research on Semax doesn't say whether it affects sleep. One user said Semax didn't make them feel wired or high, but one person's experience can't tell you how it will feel for you. Since perimenopause fog can rise and fall with sleep, track your sleep while you try it so you can see what changes.
What is the difference between Semax and Selank?
Selank is a sister peptide to Semax, and the two come up together constantly in online talk about focus and energy. This article covers Semax only: a nasal spray that sharpened attention and memory in healthy men for up to a day after one dose. Our Semax vs Selank guide compares the two for focus and anxiety.
How long does a bottle of Semax nasal spray last?
Plan on one bottle of Semax nasal spray a month, because the bottle must be used within 30 days of being made. At Rx Peps Direct, a 15 mL bottle gives 250 mcg per spray and costs $125, plus a $39 medical review on each order and $15 overnight shipping, for $179 an order. Rx Peps Direct prescribes in 28 states and Washington, DC.
