Sermorelin and tesamorelin both copy GHRH (growth hormone releasing hormone), the natural hormone that tells your pituitary to release growth hormone (review of growth hormone secretagogues).

In a small study, 10 postmenopausal women injected the full-length natural GHRH hormone twice a day for 3 months (GHRH study in postmenopausal women). Their overnight growth hormone rose 98%, nearly double, and their IGF-1 (a marker of growth hormone activity) rose 71%. Their deep belly fat, the visceral fat that sits around your organs, dropped 16%. They also walked 30 meters and climbed two flights of stairs faster. The study had no placebo group. Each woman was compared with her own starting point, and the study used natural GHRH, not sermorelin itself.

The same women's total body water rose 14%. In the first months, the scale and the tape measure can tell you different things, so judge your progress by how your clothes fit and how strong you feel.

No study has compared these peptides head to head in women, so none of them has been shown to be the best.

MK-677 is the one to skip. Beyond the FDA's heart failure concern, it shows that more lean mass on a scan does not always mean you get stronger or look more defined.

What side effects do growth hormone peptides cause?

Expect skin reactions where you inject. In the postmenopausal women's study, 70% had local skin reactions, and none had body-wide side effects (study).

Tesamorelin's label warns about raised IGF-1, fluid retention and higher blood sugar. In its trials, 5% of people on tesamorelin developed blood sugar in the diabetes range, compared with 1% on placebo (FDA label). Ask your clinical team about checking your IGF-1 and blood sugar.

The same label says not to use tesamorelin if you have active cancer, a pituitary tumor, other pituitary problems or past pituitary surgery, or if you are pregnant or planning to be. Tell your clinical team about any of these before you start a growth hormone peptide.

Most studies of growth hormone releasing drugs have been short and small, and there is no long-term data yet on cancer or survival (review). That is one more reason to start with bloodwork and a clinical team that follows up with you.
