A separate pooled analysis of data from the STEP 1, 2, 3 and 5 weight management trials included 3,377 adults with overweight or obesity in the 68-week STEP 1, 2 and 3 trials, alongside the 104-week STEP 5 trial STEP pooled analysis. Researchers checked depressive symptoms with PHQ-9, a nine-item questionnaire that asks about low mood and loss of interest, and checked suicidal thoughts and behavior with the Columbia scale, a standard interview that records intent and actions. That kind of planned, repeated measurement gives a clearer picture than scattered individual stories, though the trials enrolled people without known major mental illness at the start so they say less about women with active depression.

Why do some women still feel low or anxious?

Safety reports show that low mood does occur while women take these drugs, without showing that the drug caused it. An analysis of European EudraVigilance reports from January 2021 through May 2023 found 372 psychiatric reports among 31,444 total reports for semaglutide, liraglutide and tirzepatide, or about 1.2 in 100 reports EudraVigilance analysis. Women accounted for 65 percent of the psychiatric reports. Depression was listed most often, followed by anxiety and suicidal thoughts. Nine deaths from completed suicide and depression were recorded, mostly in men.
