Quick answer
Lipo-C injections have not been tested as a weight-loss treatment. The one Lipo-C ingredient with weight data, L-carnitine, helps people lose only about 2.7 pounds more, and only at daily doses far higher than a Lipo-C shot contains, while the B12 in Lipo-C helps if you are low on B12 but is not a fat burner. For real weight loss, prescription GLP-1 medications such as semaglutide and tirzepatide work on hunger.

Key takeaways
- Lipo-C is a name, not one fixed product, so the ingredients and amounts depend on who made it and you need to read the label.
- Rx Peps Direct, which prescribes Lipo-C, says on its own product page that Lipo-C is an add-on to diet and GLP-1 therapy, not a weight-loss drug.
- L-carnitine, the one Lipo-C ingredient with weight data, helps people lose only about 2.7 pounds more, and that takes about 2,000 mg a day by mouth, more than 900 times the weekly L-carnitine in the Defy Medical shot.
- The B12 in Lipo-C helps women who are low on B12, such as women taking metformin, women who eat little or no animal foods and women who have had weight-loss surgery, but it does not burn fat.
- GLP-1 medications such as semaglutide and tirzepatide work on hunger, and the quiz lets our clinical team see whether one fits your goal and your health history.
We found no study of Lipo-C injections as a weight-loss treatment. The only ingredient in them with real weight data is L-carnitine. In pooled trials, L-carnitine helped people lose about 1.2 kg (roughly 2.7 pounds) more than the comparison groups, at doses far higher than a Lipo-C vial contains. If you want to lose more than a couple of pounds, talk with a clinical team about a prescription weight medication such as semaglutide or tirzepatide.
What is in a Lipo-C injection?
"Lipo-C" is a name, not one fixed product. Two sellers list two different recipes:
| Ingredient | Rx Peps Direct Lipo-C | Defy Medical Lipo-C (Hallandale Bio Boost Plus) |
|---|---|---|
| B12 (cyanocobalamin) | Yes | 1 mg/mL |
| Methionine | Yes | 25 mg/mL |
| Inositol | Yes | 50 mg/mL |
| Choline | Yes | 50 mg/mL (choline chloride) |
| L-carnitine | Yes | 15 mg/mL |
| B6 (pyridoxine) | Not listed | 25 mg/mL |
| B5 (dexpanthenol) | Not listed | 25 mg/mL |
| L-arginine | Not listed | 15 mg/mL |
| Benzyl alcohol | Not listed | Yes |
| Lidocaine (a numbing medicine) | Not listed | Yes |
| Vial size | 10 mL | 10 mL |
The Rx Peps Direct listing names its ingredients but does not give amounts per mL. It lists the dose as a 1 mL shot into muscle or under the skin once a week. If you already have a vial or a quote, read the label. The ingredients and amounts depend on who made it.
Even one seller doesn't pitch it as a weight-loss drug. Rx Peps Direct prescribes Lipo-C, and its own product page says: "The standalone fat-loss evidence is weak. These are adjuncts to diet and GLP-1 therapy, not primary weight-loss drugs." In plain terms, it calls Lipo-C an add-on.
No study in this research tested a Lipo-C injection for weight loss. The weight data below comes from studies of L-carnitine on its own.
Does the L-carnitine in Lipo-C help you lose weight?
A little, at high daily doses. A 2025 review pooled eight earlier analyses covering 16,352 people. Taking L-carnitine for 8 to 30 weeks, at 150 to 4,000 mg a day, led to a small but real drop in weight, BMI and waist size. Results varied a lot from study to study.
A review of 37 trials with 2,292 people found that people taking L-carnitine lost about 1.21 kg (roughly 2.7 pounds) more than the comparison groups, plus about 2 kg of fat mass. It found no clear change in waist size or body fat percentage. When the authors kept only the best-run trials, only the drop in weight held up. They called the effect modest. In that review, the weight effect was largest at about 2,000 mg of L-carnitine a day, taken by mouth.
Now compare that with a vial. The Defy Medical formula has 15 mg/mL of L-carnitine. At 1 mL a week, that is about 15 mg of L-carnitine a week. The trials used 150 to 4,000 mg a day. The 2,000 mg a day that gave the biggest weight effect adds up to 14,000 mg a week, more than 900 times what the shot delivers. Even if your body absorbs every milligram of the injection, that small trial result does not carry over to Lipo-C.
Is the B12 in Lipo-C doing anything for my weight?
The B12 is useful if you are low on it. It is not a fat burner. B12 treats B12 deficiency, and an expert consensus lists the women more likely to run low:
- women taking metformin, which can lower B12 within about 3 months of starting
- women who eat little or no meat, eggs or dairy
- women who have had weight-loss surgery
- women with gut conditions such as atrophic gastritis or celiac disease
If you are low, shots every few weeks are not the only option. The same experts note that for people who absorb B12 poorly, either of these keeps blood levels in range as maintenance:
- 1,000 to 1,500 mcg a day by mouth
- a 1,000 mcg shot every 1 to 3 months
If you feel more energetic on a B12 shot, low B12 may be part of the reason. None of the research here shows B12 helps women who are not low lose weight. Ask your clinical team whether a B12 check makes sense for you.
What should I try instead if I want to lose real weight?
Lipo-C still costs money for that couple of pounds. Rx Peps Direct charges:
- from $35 for a 10 mL vial
- a $39 medical visit fee on each order
- $15 for FedEx overnight shipping
It prescribes in 28 states plus Washington, DC, and its vial has a 60-day use-by date.
GLP-1 medications such as semaglutide and tirzepatide work on hunger, which is what most women are fighting. On r/tirzepatidecompound, a 69-year-old who went from 265 to 200 pounds in seven months on compounded tirzepatide wrote: "The reduction in hunger, and especially the urge to overeat, made doing everything else much easier." They also tracked their food, kept their protein high, walked and did resistance training.
GLP-1s are not easy for everyone. On r/Zepbound, one person who stayed on the lowest dose of Zepbound because of side effects ended up in the ER twice in three weeks. They stopped after a little over two months. They wrote, "I lost 23 lbs and I got really great at counting calories," and "Listen to your body while it whispers, before it starts to yell." If side effects start to build, tell your clinical team early.
If you are already on semaglutide, you don't need to add Lipo-C ingredients to it. FDA says some compounding pharmacies add B12, B6, L-carnitine or NAD to semaglutide, and that "the safety and effectiveness of combining semaglutide with other ingredients has not been established."
You want the scale and your jeans to change, not a couple of pounds from a weekly vitamin shot. Our clinical team can prescribe semaglutide or tirzepatide if one fits your goal and your health history. Take the quiz so our clinical team can see whether semaglutide or tirzepatide fits your goal.

Do lipotropic injections help with belly fat?
Nothing in the research shows Lipo-C injections take off belly fat, because we found no study of Lipo-C for weight loss at all. In reviews of L-carnitine on its own, one found a small drop in waist size and another found no clear change in waist size. Both used far higher doses than a Lipo-C shot delivers.
Is Lipo-C the same as a B12 shot?
No. Lipo-C contains B12 plus methionine, inositol, choline and L-carnitine, and some versions add B6, B5, L-arginine and lidocaine, a numbing medicine. The B12 helps if you are low on it, but it is not a fat burner. For people who absorb B12 poorly, 1,000 to 1,500 mcg a day by mouth or a 1,000 mcg shot every 1 to 3 months keeps blood levels in range.
Can you take Lipo-C with semaglutide or tirzepatide?
Rx Peps Direct sells Lipo-C as an add-on to GLP-1 therapy, but if you are already on semaglutide, you don't need to add Lipo-C ingredients to it. The FDA says some compounding pharmacies add B12, B6, L-carnitine or NAD to semaglutide, and that the safety and effectiveness of combining semaglutide with other ingredients has not been established.
How much do Lipo-C injections cost?
At Rx Peps Direct, Lipo-C starts at $35 for a 10 mL vial, plus a $39 medical visit fee on each order and $15 for FedEx overnight shipping, so one order starts at $89. Rx Peps Direct prescribes in 28 states plus Washington, DC. Its vial has a 60-day use-by date, so at one 1 mL shot a week, the date runs out before the tenth dose.
