Quick answer
Oral DHEA did not improve low libido more than placebo, though it raises blood testosterone and estrogen and vaginal DHEA helps pain with sex. Oral DHEA was not tested for low libido in perimenopause itself.

Key takeaways
- In the libido trial, satisfying sexual events, sexual self-rating, well-being and menopause quality of life showed no difference between DHEA and placebo.
- More women taking oral DHEA reported acne and increased hair growth.
- Vaginal DHEA is not indicated for low desire or HSDD, and estrogen conversion leaves safety questions for hormone-receptor-positive breast cancer.
Oral DHEA did not improve low libido more than placebo in the main 26-week trial 26-week trial summary. Oral DHEA does raise testosterone and estrogen in the blood 2025 combined analysis, and vaginal DHEA is approved for pain with sex FDA review. That split explains why a higher lab value does not promise higher desire.
What happened when women with low libido took oral DHEA?
Researchers gave 93 postmenopausal women with low libido either 50 mg of oral DHEA each day or placebo and measured change at 26 weeks 26-week trial summary. Women taking DHEA reported no more satisfying sexual events than women taking placebo. Scores for sexual self-rating, general well-being and menopause quality of life also showed no significant difference between groups.
More women in the DHEA group reported acne and increased hair growth. A small study of low desire with distress, called hypoactive sexual desire disorder or HSDD, described earlier DHEA results as scarce and inconsistent HSDD gender-differences abstract. No study in this set enrolled women in perimenopause for low libido.
If DHEA raises testosterone, why didn't desire improve?
A 2025 combined analysis of 21 trials found oral DHEA raised serum testosterone and estradiol in postmenopausal women, with larger testosterone rises at doses of 50 mg per day or more 2025 combined analysis. Estradiol is a form of estrogen. Those were blood measurements.
The libido trial measured lived experience, meaning satisfying events and how women rated their own sexuality, and found no advantage for DHEA. A higher hormone number on paper does not automatically change how desire feels day to day. The research shows a lab effect without a proven desire benefit.
What is vaginal DHEA actually approved for?
Vaginal DHEA, sold as prasterone or Intrarosa, is a small insert you place in the vagina. The FDA approved it in November 2016 for moderate to severe pain with sex due to menopause, a symptom of vaginal thinning, after four 12-week trials FDA review. The approval covers pain, which clinicians call dyspareunia.
The maker states the product is not indicated for sexual dysfunction and it was not studied in women diagnosed with HSDD maker efficacy page. In a later review of trial data, total scores on a sexual function survey rose 62 percent from baseline at 12 weeks, but that review was not designed to prove a difference and it excluded women with an HSDD diagnosis. Because DHEA converts to estrogen in the body, questions remain about use in women with hormone-receptor-positive breast cancer, and higher DHEA blood levels are linked with higher breast cancer risk breast cancer safety summary.
