Quick answer

Peptide secretagogues such as sermorelin, ipamorelin and CJC-1295 tell your pituitary gland to release more of your own growth hormone, and one CJC-1295 shot keeps that boost going for six days or more. Peptide bioregulators such as Thymalin and Epithalamin are thymus and pineal gland peptides sold on promises of resetting your organs and helping you live longer, but the backing comes mostly from the Russian group that developed them and was aimed at people over 60. If you want more energy, better recovery and a body that answers your workouts again, secretagogues are the option with measured results in people.

Key takeaways

  • Secretagogues copy a signal your brain already sends, so the growth hormone still comes from your own pituitary, in its normal pulses, with your body's own brakes still working.
  • One CJC-1295 shot raises your own growth hormone for six days or more and keeps IGF-1 (the growth factor your liver makes in response) up for over a week, and repeat doses can keep it up for about four weeks.
  • Thymalin and Epithalamin are sold on big promises about longer life, but the research comes mostly from the group that developed them and was done in people over 60, not women in their 30s, 40s and early 50s.
  • Blood sugar is the main thing to watch on a growth hormone peptide, so give your clinical team your full health history before you start.
  • Compounded secretagogues run about $125 to $225 a month: Elara lists sermorelin at $175 for one month or $125 a month on a six-month plan, and TelaCure lists ipamorelin at $225 for a 30-day vial, prescribed only after a licensed provider review.

Peptide secretagogues such as sermorelin, ipamorelin and CJC-1295 signal your pituitary gland to release more of your own growth hormone, and placebo-controlled trials of CJC-1295 have measured that rise. Peptide bioregulators such as Thymalin and Epithalamin are peptides from the thymus and pineal gland, promoted to normalize how organs work and to extend life, and the evidence for them comes mostly from one Russian research group. The real difference is not natural versus synthetic. It is how clearly each one's effect on the body has been measured.

If you want more energy, better recovery and a body that responds to your workouts again, the secretagogues are where the measured human data sits.

How do peptide secretagogues like sermorelin work?

Secretagogues work by copying a signal your brain already sends. Your brain makes a hormone called growth hormone-releasing hormone (GHRH), which tells your pituitary gland to make growth hormone and release it in pulses. Growth hormone helps build muscle and break down fat, according to the tesamorelin prescribing label.

Sermorelin and tesamorelin are lab-made versions of that GHRH signal. A 2017 review of growth hormone secretagogues describes them as switching on the same GHRH receptor your own hormone uses.

CJC-1295 is a long-acting version of GHRH. In two randomized, placebo-controlled trials in healthy adults aged 21 to 61, one injection raised average growth hormone 2 to 10 times for 6 days or more. It also raised IGF-1 1.5 to 3 times for 9 to 11 days. IGF-1 is the growth factor your liver releases when growth hormone tells it to. The drug's estimated half-life (the time it takes for half of a dose to clear) was 5.8 to 8.1 days. After multiple doses, IGF-1 stayed above where people started for up to 28 days.

Secretagogues make your pituitary release growth hormone in its normal pulses, and your body's own feedback can still put the brakes on. The 2017 review says this may keep growth hormone from climbing too high.

That makes "synthetic" a misleading label. These peptides are made in a lab, but they copy or mimic a signal your body already sends. The growth hormone itself still comes from you.

What are peptide bioregulators like Thymalin and Epithalamin?

Bioregulators are peptides taken from glands, sold on the claim that they reset how whole organ systems work. Thymalin comes from the thymus, a gland in the chest that trains immune cells. Epithalamin comes from the pineal gland in the brain. Researchers in St. Petersburg and Kiev call them peptide bioregulators and say they normalize the heart, hormone, immune and nervous systems.

Their headline study followed 266 people over 60 for 6 to 8 years, with the peptides given in the first 2 to 3 years. Compared with a control group, the authors reported death rates 2.0 to 2.1 times lower with Thymalin, 1.6 to 1.8 times lower with Epithalamin and 2.5 times lower with both. In a group treated every year for 6 years, death rates were 4.1 times lower.

Those numbers are dramatic, but the study abstract does not say whether people were randomly assigned or given a placebo. Those are the two things that let you trust a result like this.

Most of the clinical record comes from the same group. A 2014 review of bioregulator trials covering Timalin, Thymogen, Vilon, Epithalamin, Prostatilen, Cortexin and Retinalamin was written by the authors about their own long-term studies.

The longevity work was also done in people over 60, and the authors conclude the bioregulators are meant for that age group. It was not done in women in their 30s, 40s and early 50s.

How do secretagogues and bioregulators compare side by side?

Secretagogues (sermorelin, ipamorelin, CJC-1295) Bioregulators (Thymalin, Epithalamin)
What it is Lab-made copies or mimics of the body's growth hormone signals (2017 review) Thymus and pineal peptides
What it targets The pituitary and growth hormone Claimed to normalize many body systems at once, including heart, hormone, immune and nervous system function and metabolism (St. Petersburg and Kiev study)
Best human data Randomized, placebo-controlled, double-blind trials of 28 and 49 days in healthy adults aged 21 to 61 measured growth hormone and IGF-1 rises with CJC-1295 (trial) A 266-person study in people over 60 reported lower death rates, with no randomization or placebo described in the abstract
Who ran the research Drug companies, universities and FDA-reviewed label studies Mostly the group that developed the peptides (2014 review)
FDA status Tesamorelin (Egrifta) is FDA approved only to reduce excess belly fat in adults with HIV-related lipodystrophy, and its label says it is not for weight loss. Sermorelin and other GHRH analogs are not FDA approved but are available by prescription through compounding pharmacies. Not covered by the studies above

No study has compared the two groups head to head, so neither one is better overall. The difference is in what each has measured and in whom.

What side effects should you watch for on a growth hormone peptide?

Secretagogues have been well tolerated in the studies done so far, according to the 2017 review. Blood sugar is the main thing to watch, because these peptides can make your body less sensitive to insulin. The same review notes that few long, rigorous trials exist, and there are no data yet on cancer or death rates over years of use.

FDA has reported serious adverse events with CJC-1295, including a faster heart rate and a body-wide vasodilatory reaction (blood vessels widening, which shows up as flushing), and says clinical data are limited. Ask your clinical team what to watch for.

FDA lists ipamorelin acetate under category 2 for section 503B compounding, meaning it may present significant safety risks. It has been on that list since September 29, 2023. FDA cites possible immune reactions from impurities and a published study in which ipamorelin given by IV for gut motility was linked to serious adverse events, including death. FDA says it lacks safety information for some other injectable routes.

The approved secretagogue's label shows the kind of effects this drug class can bring. The tesamorelin label lists fluid retention (swelling, joint aches and carpal tunnel syndrome) that passes or goes away once you stop. In its trials, diabetes (an HbA1c of 6.5% or higher) developed in 5% of people on tesamorelin vs 1% on placebo. These numbers belong to tesamorelin only.

The same label says not to use tesamorelin during pregnancy or with active cancer, and says IGF-1 levels need periodic checks. Tell your clinical team your full health history before starting any growth hormone peptide.

How much does a secretagogue like sermorelin or ipamorelin cost?

Compounded secretagogues run from about $125 to $225 a month at the providers below, depending on the peptide and how many months you pay for upfront.

  • Elara (a women's telehealth brand) lists compounded sermorelin at $175 for one month, $477 for three months ($159 a month) or $750 for six months ($125 a month).
  • TelaCure lists compounded ipamorelin at $225 a month for a 5 mg injection vial, a 30-day supply, prescribed only after a licensed provider review.

These peptides are not FDA approved, so they are prescribed through compounding pharmacies, which make the medicine to order for you.

FAQ

Is sermorelin the same as HGH?

No. Sermorelin is a lab-made copy of GHRH, the signal your brain sends to tell your pituitary gland to make growth hormone. The growth hormone itself still comes from your own body, released in its normal pulses. Your body's own feedback can still put the brakes on, which may keep growth hormone from climbing too high.

Is sermorelin FDA approved?

No. Sermorelin and other copies of the GHRH signal are not FDA approved, but they are available by prescription through compounding pharmacies, which make the medicine to order for you. A related peptide, tesamorelin (Egrifta), is FDA approved only to reduce excess belly fat in adults with HIV-related lipodystrophy (a change in where the body stores fat), and its label says it is not for weight loss.

How long does CJC-1295 stay in your system?

CJC-1295 lasts about a week, with half of a dose clearing in roughly 6 to 8 days. In healthy adults, one injection raised growth hormone for 6 days or more and raised IGF-1, the growth factor your liver makes in response, for 9 to 11 days. With repeated doses, IGF-1 stayed above starting levels for up to 28 days.

Do growth hormone peptides raise blood sugar?

They can. Growth hormone peptides can make your body less sensitive to insulin, so blood sugar is the main thing to watch. In the trials of tesamorelin, the approved peptide in this group, more people developed diabetes on the drug than on placebo. Ask your clinical team what to watch for and share your full health history before starting.
