Quick answer

If you are losing muscle after 40, neither sermorelin nor testosterone has shown a muscle gain in women at the doses guidelines support. Sermorelin raises your own growth hormone within weeks and thickened women's skin, for $125 to $175 a month at Elara, checked October 2026. Testosterone suits women after menopause whose desire has also dropped.

Key takeaways

  • Neither sermorelin nor testosterone has shown a muscle gain in women at the doses guidelines support, and nobody has compared the two head to head in women, so choose by what else you want.
  • Sermorelin raises your own growth hormone within minutes of a nightly shot, and over four months it made women's skin thicker, while the extra muscle showed up in men.
  • Testosterone added muscle in women only at a high weekly injected dose, and guidelines back it for women mainly for low desire after menopause, not for muscle or general well-being.
  • Sermorelin's only side effect was a short-lived rise in blood fats, while testosterone can raise red blood cell levels and add a little body or facial hair, so levels get checked at the start, after 3 to 6 weeks and every 6 months.
  • Sermorelin costs $125 to $175 a month at Elara depending on plan length, and women's testosterone cream starts at $60 a month plus membership at Hone Health, with $65 lab tests, both checked October 2026.

You want back the muscle you have lost since your 40s, and no trial has compared sermorelin and testosterone head to head in women. Sermorelin is a compounded peptide that prompts your own pituitary to release growth hormone. In a 16-week trial of 10 women and 9 men aged 55 to 71, a sermorelin-type peptide thickened women's skin but raised lean mass only in the men. Testosterone has no product made for women at female doses in the US, according to the Endocrine Society guideline. It raised lean mass by 1.8 kg only at the highest injected dose in a 24-week trial of 71 women, and the 2019 Global Consensus found no significant muscle gain at normal female doses.

Is sermorelin or testosterone better for building muscle in women?

Neither drug has shown a muscle gain in women at a dose the guidelines support, and no trial has compared them. Choose by what else you want. Testosterone fits if low desire after menopause is part of it. Sermorelin fits if you want to raise your own growth hormone without taking a sex hormone.

Sermorelin Testosterone
How it works A lab-made piece of growth hormone releasing hormone. In a 16-week trial, nightly shots of a sermorelin-type peptide released growth hormone within 10 minutes and raised IGF-1 (a growth factor driven by growth hormone) within 2 weeks. An androgen (a male-type hormone). In a 24-week trial of 71 menopausal women, weekly injections raised blood testosterone in step with the dose.
Legal status Sellers offer it as a compounded injection. Elara's sermorelin is prepared by Greenwich Rx, a compounding pharmacy. Compounded drugs are not FDA-approved, and FDA does not check their safety, effectiveness or quality before they are sold. The Endocrine Society guideline (2014) says testosterone products made for women at female doses are not available in the US. Compounded versions are not FDA-approved, and FDA does not check their safety, effectiveness or quality before they are sold.
Muscle evidence in women In the 16-week trial, lean mass rose in the men only, not the women. At normal female doses, the 2019 Global Consensus found no significant effect on lean mass, body fat or muscle strength. In a 24-week trial, the gain in women (1.8 kg lean mass) came only at the highest injected dose.
Side effects In the 16-week trial, the only side effect was a temporary rise in blood fats, which cleared by the end of the study. At normal female doses, some women get mild increases in acne and body or facial hair, according to the 2019 Global Consensus.
Monthly price $125 to $175 a month at Elara, depending on plan length, checked October 2026. Compounded cream from $79 a month at Zepym, checked October 2026.
Who it fits The 16-week trial studied women and men aged 55 to 71. Our clinical team can prescribe sermorelin if it fits your goal. The 2019 Global Consensus backs it only for postmenopausal women with low sexual desire that causes distress (hypoactive sexual desire disorder). It found too little data to recommend testosterone before menopause.

What women share

We read 52 low-libido threads in r/Menopause, r/Perimenopause and r/TwoXChromosomes from October 2025 to October 2026. 18 of 52 mentioned testosterone. These were threads about desire, not muscle.

Why is it harder to regain muscle after 40?

Your growth hormone system winds down with age. The researchers behind the sermorelin-type peptide trial say that weakening may be behind the changes in body composition and metabolism that come with getting older.

Testosterone falls with age too, and the authors of the 24-week testosterone trial note that this drop in older women has been linked to frailty. They also point to the SWAN study of midlife women, which found more physical limitation after surgical menopause than after natural menopause.

Growth hormone shots themselves caused side effects that limited their use in older adults, which is why researchers tested the releasing-hormone route that sermorelin uses.

Sermorelin aims at the first drop and testosterone at the second, so the choice depends on which hormone story fits you. See all our options for body goals.

What does the research show for each?

Sermorelin-type peptide. In a single-blind trial of 10 women and 9 men aged 55 to 71, everyone self-injected saline nightly for 4 weeks, then the peptide nightly for 16 weeks. In women, overnight growth hormone rose and IGF-1 went up within 2 weeks. IGF-1 stayed up for 12 weeks, then drifted back toward baseline by week 16.

In that trial, skin got thicker in both women and men. Lean mass, well-being and libido improved in the men only, and sleep did not change in either group. For more on how sermorelin relates to body changes, read our guide to sermorelin for muscle and fat loss.

Testosterone injections. A placebo-controlled trial enrolled 71 menopausal women with low testosterone who had had a hysterectomy (some also had their ovaries removed). All were on estrogen through the skin, and they took one of four weekly testosterone doses or placebo for 24 weeks. Only the highest dose beat placebo: 1.8 kg more lean mass, 27 watts more chest-press power and 57 watts more loaded stair-climb power. Fat mass did not change, and walking speed and unloaded stair climbing did not improve.

The 2019 Global Consensus does not recommend injections. It groups them with pellets as products that push testosterone above the normal premenopausal range. In the trial's highest-dose group, average blood testosterone was 210 ng/dl, against 19 ng/dl on placebo.

The Endocrine Society guideline says testosterone raises lean mass and may lower fat in women, but less than in men. It recommends against general testosterone use for metabolic health, bone health or general well-being.

What are the side effects of each?

Treatment and source Side effects and cautions
Sermorelin-type peptide (16-week trial, 10 women and 9 men aged 55 to 71) The only side effect was a temporary rise in blood fats, which cleared by the end. Blood pressure, weight, fasting glucose and insulin did not change.
Testosterone injections (24-week trial, 71 women) Red blood cell levels (hemoglobin and hematocrit) rose with the dose, significantly at the highest dose. Body and facial hair scores rose slightly at the two highest doses. Acne, clitoral size and oily skin did not change, and there were no serious testosterone-related events.
Testosterone at normal female doses (2019 Global Consensus) Some women get mild acne and extra body or facial hair, but not hair loss, clitoral enlargement or voice change. Testosterone pills are not recommended because they worsen cholesterol. There are no safety data past 24 months, and the consensus urges caution after hormone-sensitive breast cancer.
Testosterone monitoring (Endocrine Society guideline) If testosterone is prescribed, the guideline suggests checking levels at the start, again after 3 to 6 weeks and then every 6 months, and watching for signs of too much androgen.
Any compounded drug (FDA) Poor compounding can produce a drug with too much or too little active ingredient.

Ask your clinical team what to watch for.

FAQ

how long does sermorelin take to work for women

Sermorelin starts working on your hormones within minutes and weeks. In a study of women and men aged 55 to 71, each nightly shot released growth hormone within 10 minutes, and IGF-1 (a growth factor driven by growth hormone) rose within 2 weeks. Women's skin got thicker over 16 weeks of nightly shots.

will testosterone give women facial hair

Testosterone can add a little facial or body hair. In a 24-week study of 71 women on weekly testosterone shots, hair scores rose slightly at the two highest doses, while acne, oily skin and clitoral size did not change. If testosterone is prescribed, the Endocrine Society suggests checking your levels at the start, after 3 to 6 weeks and every 6 months, and watching for signs of too much androgen.

does sermorelin help women lose belly fat

Sermorelin has not shown fat loss in women. In a 16-week study of nightly shots in women and men aged 55 to 71, body weight did not change, and women's body composition stayed the same while their skin got thicker. Our guide to sermorelin for muscle and fat loss covers this in more detail.

can women get testosterone prescribed online

Yes, some telehealth sellers prescribe compounded testosterone cream for women online. Zepym lists it from $79 a month and says a live video visit is required before prescribing because testosterone is a controlled substance, and Hone Health lists it from $60 a month plus membership, with $65 lab tests, both checked October 2026. Compounded versions are not FDA-approved, and the US has no testosterone product made for women at female doses.
