Quick answer

Thymosin alpha-1 is not proven to ease fatigue or joint pain after a virus. In lab tests, thymosin alpha-1 nudged worn-out immune cells back toward normal, but nobody has shown that it gives tired people more energy or repairs sore joints. In the US, thymosin alpha-1 is not FDA-approved and is sold only as a compounded prescription injection, and the FDA has flagged safety concerns with compounded versions.

Key takeaways

  • Thymosin alpha-1 is not proven to relieve fatigue or joint pain after a virus, and in the US it is not FDA-approved.
  • Thymosin alpha-1 can shift immune markers in people who are sick, but nobody has shown that people with lingering fatigue feel more energetic on it.
  • If your joint pain started after a virus, talk to your clinical team first, because thymosin alpha-1 labels outside the US warn against it for people with autoimmune diseases, and post-viral joint pain can sometimes be autoimmune.
  • Thymosin alpha-1 most often causes irritation, redness or soreness where the needle goes in, and the FDA warned that compounded versions could make your body form an immune reaction against the peptide itself.
  • Hot or Nothing does not offer thymosin alpha-1, but your clinical team can look at your fatigue and recovery and see whether a plan with NAD+, MOTS-c, sermorelin, BPC-157 or another treatment fits your goal.

Thymosin alpha-1 has not been shown to relieve post-viral fatigue or joint pain in people. The closest research is a lab study of blood cells from 10 people with long COVID, where the peptide appeared to push their immune cells back toward a normal response. No study in the research looked at joint recovery. In the US, thymosin alpha-1 is not FDA-approved and is only available as a compounded medication, and FDA's 2024 compounding review flagged possible safety risks with compounded versions.

What is thymosin alpha-1, and why do people try it after a virus?

Thymosin alpha-1 is a peptide, a short chain of 28 amino acids, made by the thymus. The thymus is the gland where your T cells mature, and T cells are the immune cells that clear viruses.

It is an approved prescription drug in some countries. Singapore registers it as Zadaxin for chronic hepatitis B. The dose is 1.6 mg injected under the skin twice a week for 6 to 12 months.

Some people with long-lasting fatigue hear about it from integrative doctors and peptide forums. One person on r/Peptides said they have CIRS and ME/CFS and are mostly bedridden. Their integrative doctor suggested several peptides, including thymosin alpha-1. They wrote: "I don’t want to do anything super risky, but am also desperate to get well and have my life back."

Approval for hepatitis B tells you thymosin alpha-1 has a safety record in that use. It does not tell you it treats fatigue or joint pain. The rest of this page shows what the research covers.

What does the research show for long COVID and post-viral fatigue?

Who was studied What was done What the study measured What happened
10 people with long COVID who had been hospitalized and were still unwell weeks to months later (long COVID study) Their blood cells were tested in a lab with and without thymosin alpha-1 How their immune cells responded in the lab The peptide appeared to push their immune cells back toward a normal response. Nobody received it as a treatment, and fatigue was not measured.
20 people with HIV whose immune systems had not recovered on their usual medication, 19 of them men, median age 48 (HIV study) 1.6 mg injections under the skin for 24 weeks T cell counts and side effects Their share of naive CD4 T cells (fresh T cells that haven't fought an infection yet) rose from 17.2% to 41.1%. The rise in total CD4 count was not statistically significant at 24 weeks. There were no severe side effects. Energy and how people felt were not measured.
Adults with sepsis in the TESTS trial, a phase 3 placebo-controlled study and the largest trial here Thymosin alpha-1 or placebo Deaths within 28 days, and side effects 23.4% died on thymosin alpha-1 and 24.1% on placebo, so no clear effect on survival. At least one side effect showed up in 66.4% on thymosin alpha-1 and 67.6% on placebo.
People with COVID-19, pooled in a 2023 meta-analysis listed in FDA's review Thymosin alpha-1 in earlier studies, combined Deaths from COVID-19 No statistically significant effect on deaths.

The pattern across these studies is that thymosin alpha-1 shifts immune markers in sick people. No study here shows that people with lingering fatigue felt more energetic on it. Better lab numbers and feeling better are separate results, and only the first has been studied.

Can thymosin alpha-1 help joint pain after a virus?

No study in this research tested thymosin alpha-1 for joint pain or joint recovery after a viral illness. Anyone telling you it repairs joints is going beyond the evidence.

There is also a reason for caution. According to FDA's review, labels for thymosin alpha-1 products sold outside the US carry warnings and contraindications for people with autoimmune diseases. That matters here, because joint pain that starts after a virus can sometimes be autoimmune.

If your joints are swollen, hot or stiff in the morning, tell your clinical team before you try any treatment that acts on your immune system. They can check what is driving the pain first.

Is thymosin alpha-1 safe, and can you get it in the US?

In most clinical studies, at doses of 1 to 16 mg under the skin for up to 12 months, the most common side effects were irritation, redness or discomfort where the needle went in, FDA's review notes.

The Zadaxin label says it should be considered off-limits for people whose immune systems are being deliberately suppressed, such as organ transplant patients, unless the benefits clearly outweigh the risks. FDA's review also lists flares in ALT, a liver enzyme, in people with chronic hepatitis B, and changes in TSH, a thyroid hormone, in people with chronic hepatitis C.

Compounded versions raise their own questions. FDA has said compounded thymosin alpha-1 "may pose a significant risk for immunogenicity" for certain routes of use, which means your body could form an immune reaction against the peptide itself. FDA also said the safety information is too thin for it to understand the full risk. The request to add thymosin alpha-1 to FDA's compounding list was withdrawn, and FDA kept its risk note.

In the US, thymosin alpha-1 is not FDA-approved. Online clinics offer it as a compounded injection by prescription. Promise lists a 1 mg/mL, 5 mL vial at $229 for the first shipment, then $279 every 28 days until you cancel.

Can Hot or Nothing prescribe thymosin alpha-1?

Hot or Nothing does not offer thymosin alpha-1. Our energy and aging plans include NAD+, MOTS-c, ipamorelin and CJC-1295. Our healing and recovery plans include sermorelin, BPC-157 and TB-500.

Tell your clinical team how long the fatigue has lasted, what makes it worse, which joints hurt and any blood work you already have. That gives them what they need to decide whether one of our treatments fits your goal.

Your clinical team decides whether a treatment fits your health history. If something else needs to be checked first, they will tell you.

FAQ

thymosin alpha-1 side effects in women

The most common side effects of thymosin alpha-1 are irritation, redness or discomfort where the needle goes in. The research did not look at women separately, but labels for products sold outside the US carry warnings for pregnant and breastfeeding women. Other reported problems include flares in ALT, a liver enzyme, in people with chronic hepatitis B, and changes in TSH, a thyroid hormone, in people with chronic hepatitis C. The FDA also said compounded versions could cause your body to form an immune reaction against the peptide itself.

can you take thymosin alpha-1 if you have an autoimmune condition

Thymosin alpha-1 may not suit you if you have an autoimmune condition, because labels for products sold outside the US carry warnings and contraindications for people with autoimmune diseases. Thymosin alpha-1 acts on the immune system, and the Zadaxin label says it should be considered off-limits for people whose immune systems are being deliberately suppressed, such as organ transplant patients, unless the benefits clearly outweigh the risks. Tell your clinical team about any autoimmune diagnosis, or joints that are swollen, hot or stiff in the morning, before you try a treatment that acts on your immune system.

how often do you inject thymosin alpha-1

For its approved use in chronic hepatitis B in Singapore, thymosin alpha-1 (sold as Zadaxin) is injected under the skin at 1.6 mg twice a week for 6 to 12 months. Most clinical research also used 1.6 mg under the skin. There is no tested schedule for fatigue or joint pain, because thymosin alpha-1 has not been proven for either.

how much does thymosin alpha-1 cost per month

Promise lists a 1 mg/mL, 5 mL vial of compounded thymosin alpha-1 at $229 for the first shipment, then $279 every 28 days until you cancel. Thymosin alpha-1 is not FDA-approved in the US, so online clinics offer it only as a compounded injection by prescription. Hot or Nothing does not offer thymosin alpha-1.
