A longer follow-up showed the same pattern after a larger loss. In an extension with 327 adults, average loss after 68 weeks on semaglutide was 17.3%. One year after stopping, they had regained 11.6 percentage points, or about two-thirds of the loss. They kept a net loss of 5.6% from the start, and weight was still rising at the final visit. How fast does regain happen, and do health gains fade too?
Regain starts fast and then slows. Across six trials with 3,236 participants, people had regained about 60% of lost weight a year after stopping. The model estimated regain would level off at about 75.3% of the loss, with about half of that long-term regain reached by 23 weeks. 2026 meta-regression
A separate review estimated regain at about 0.4 kg a month, which would bring weight back to the starting point by about 1.7 years after stopping. No large trial has tracked weight longer than a year off treatment, so longer timelines are projections. BMJ review
Health measures moved with weight. In the STEP 1 follow-up, blood pressure, blood sugar and cholesterol improvements seen during treatment moved back toward starting levels a year after stopping.
