Quick answer
Prescription tretinoin is the anti-aging skincare product with the best track record for women over 40, because it smooths both fine lines and deeper wrinkles. Tretinoin can sting for the first few weeks, and most women should give it about four months to show a clear change. Wear sunscreen and moisturizer every day with tretinoin, and add a vitamin C and peptide serum or choose a gentler retinol depending on how sensitive your skin is.

Key takeaways
- Prescription tretinoin has the best track record for smoothing fine lines and deeper wrinkles after 40, and it is the only prescription retinoid known to work on sun-related wrinkles.
- Over-the-counter retinol is easier on sensitive skin, but it does less than tretinoin.
- A vitamin C and peptide serum can make crow's feet and forehead lines look softer and skin look brighter within about a month, though the early results come from the brand that makes it.
- Daily sunscreen and an ordinary moisturizer can improve fine lines on their own, so both belong in every routine.
- Tretinoin can sting, redden or peel in the first few weeks, starts to change deeper wrinkles in about a month, and shows a clear difference by about four months that keeps building for up to two years.
For most women over 40, prescription tretinoin is the anti-aging product with the strongest proof. In randomized trials it improved both fine and coarse wrinkles more than a plain cream did. Daily sunscreen and moisturizer matter more than most lists admit. A vitamin C and peptide serum or a gentler retinol can fit around tretinoin, depending on how sensitive your skin is.
Which anti-aging product has the most proof for wrinkles after 40?
Tretinoin has the most proof. A review of eight randomized trials included 1,361 people, with median ages from 29 to 76 across the studies, followed for 16 weeks to 2 years. Tretinoin improved fine and coarse wrinkles from sun damage more than the same cream without tretinoin.
Most of the people in these trials were women. A second review of seven randomized trials found that four studied only women, and women made up most of the people in the other three. Every sign of sun damage improved except rough texture.
Tretinoin also stands apart from the other prescription retinoids, the skin creams related to vitamin A. Six classes of topical retinoid are approved today. The first review's authors say tretinoin is the only one that works on sun-related aging and wrinkles.
Our clinical team can prescribe tretinoin if it fits your skin and your goal.
How does tretinoin compare with retinol, peptide serums and sunscreen?
| Product | What the research showed | Kind of evidence | How it tends to feel |
|---|---|---|---|
| Prescription tretinoin | Improved fine and coarse wrinkles more than a plain cream | Randomized trials, collected in two reviews (8 trials, 7 trials) | Burning, redness, itching or peeling can show up in the first weeks |
| Over-the-counter retinol | Smaller effect than tretinoin | One study comparing it with tretinoin | Slight redness and irritation |
| Vitamin C and peptide serum | Crow's feet scores fell 9% and forehead lines 11% after 28 days | Open studies with brand-affiliated authors and no comparison group | 97.7% of women in a large observational study rated how their skin tolerated it as good or very good |
| Sunscreen plus moisturizer | Many people's fine wrinkles improved | Comparison group in the tretinoin label trials | Not measured in these studies |
None of these studies tested tretinoin directly against a peptide serum.
Retinol is easier on your skin, but it does less. In one study, retinol had effects on skin receptors similar to tretinoin's and caused only slight redness and irritation. Its effect on the skin was also smaller. That trade can make sense if your skin reacts to everything.
The vitamin C and peptide serum looks promising, with early results. The serum contains 10% vitamin C, rice and lupin peptides, and hyaluronic acid. In open studies of 40 women using it twice a day, crow's feet scores fell 9%, forehead lines 11% and the fold from nose to mouth 5.2% after 28 days. In one of those studies, the number of wrinkles dropped 11.5% by day 29. None of these studies had a comparison group, and scientists from the company that makes the serum were among the authors.
A larger observational study followed 1,382 women in nine countries, with an average age of 48.5. After a month on the same serum, 63% saw improvement in forehead lines and 64% in crow's feet. The authors included scientists from L'Oréal and Vichy, and this study also had no comparison group.
Sunscreen and moisturizer do more than people think. The label for Renova, a 0.02% tretinoin cream, describes trials where the comparison group used no tretinoin. Many of those people saw their fine wrinkles improve using sunscreen, protective clothing and an ordinary moisturizer. Whatever else you use, these two belong in your routine.
How long does tretinoin take to work, and what does the first month feel like?
Give it about four months. In some studies, coarse wrinkles started to change in about a month. The improvement became significant by four months and kept building for up to two years.
The first few weeks can sting. Burning, redness, itching or peeling is common early on. Doctors call this a retinoid reaction, and it happens more often with tretinoin than with retinol.
Most of that irritation is mild to moderate. Across 34 studies, there were no big differences between retinoids in how many people stopped because of irritation. How people used the cream likely explained more of the difference than which formula they used. Your clinical team can tell you how much to use and how often to start.
A microencapsulated tretinoin, which comes in 0.04% or 0.1%, releases the drug slowly. That can make it gentler on your skin. Retinoids can also make your skin burn more easily in the sun when you start, so wear sunscreen from day one.
The Renova label says to use tretinoin under medical supervision, with sunscreen of at least SPF 15 and protective clothing. You may lose some of the effect 12 weeks after you stop. What happens with ongoing irritation past 52 weeks of use has not been studied.
Which anti-aging routine fits my skin?
- You want the biggest change in your lines. Build your routine around nightly tretinoin. Add daily sunscreen and a plain moisturizer, and give it four months before you judge it.
- Your skin is sensitive or reactive. Ask your clinical team about a gentler start, such as microencapsulated tretinoin, or about retinol. Retinol is easier on your skin but does less.
- You want more glow and smoother texture on top of tretinoin. A vitamin C and peptide serum is an option. Ask your clinical team how to fit it into your routine.
- Your makeup might be making lines look deeper. In a Reddit thread, someone worried about sagging got this top reply: "Your foundation looks really heavy." Another commenter added: "I switched to a tinted spf when I want to have a full makeup look, otherwise I try to stick to concealer only where needed."
- Heaviness or sagging around your mouth and jaw bothers you most. The studies here measured wrinkles, spots and texture. Tell your clinical team about sagging so your plan targets it.
- You want to know what else is available. Our skin plans also include GHK-Cu copper peptide and NAD+ face cream. Your clinical team can talk through whether either belongs in your routine.
If you want your lines smoother and your skin brighter after 40, take the quiz. Our clinical team can see whether prescription tretinoin fits your skin and build a plan around it.

can you use vitamin c serum and tretinoin together
Yes, a vitamin C and peptide serum can fit into a tretinoin routine if you want more glow and smoother texture. Women using one such serum twice a day saw softer crow's feet and forehead lines within about a month, though those early results come from the brand that makes it. Ask your clinical team how to fit the serum around your tretinoin.
what strength of tretinoin should I start with over 40
There is no single starting strength for every woman over 40, so your clinical team picks one based on your skin and your goal. If your skin is sensitive, ask about microencapsulated tretinoin, which comes in 0.04% or 0.1% and releases the drug slowly so it can feel gentler. Your clinical team can also tell you how much to use and how often to start.
does tretinoin help sagging skin
Tretinoin is known for smoothing fine lines and deeper wrinkles, but the research measured wrinkles, spots and texture, not sagging. If heaviness around your mouth and jaw bothers you most, tell your clinical team so your plan targets it. Heavy foundation can also make lines look deeper, so some women switch to a tinted sunscreen and spot concealer.
is it safe to use tretinoin long term
Tretinoin has been used safely in women for up to two years in research, and the improvement in wrinkles kept building over that time. The Renova label says to use tretinoin under medical supervision, with sunscreen of at least SPF 15 and protective clothing, and notes that ongoing irritation past 52 weeks has not been studied. You may lose some of the effect about 12 weeks after you stop, so talk with your clinical team about keeping it in your routine.
