Quick answer
The best tretinoin strength for a beginner over 40 who wants to soften fine lines and sun damage is 0.025% cream, used a few nights a week. The low 0.025% strength smooths sun-damaged skin as well as the stronger 0.1% cream, with less redness and peeling, so it is easier to stick with. Tretinoin takes months to show results, and you can move up to a higher strength later if your skin needs it.

Key takeaways
- Start with 0.025% tretinoin cream a few nights a week, because it improves sun damage as well as the 0.1% cream with less redness and peeling.
- You can move up to a stronger tretinoin later, but if your skin is improving on the low strength, you can stay there.
- The strength of tretinoin matters more than whether it comes as a cream or gel, and the 0.1% cream is the harshest option.
- Keep your tretinoin routine plain: wash gently, apply a thin layer in the evening, use a moisturizer, wear SPF 15 or higher in the sun and tell your clinical team about your other products.
- Tretinoin takes months to work and is aimed at fine lines, uneven tone, sun spots and rough texture, not sagging or lost volume.
If you're new to tretinoin and want it for fine lines and sun damage, start with the lowest common cream strength, 0.025%, a few nights a week. In a 48-week photoaging trial, 0.025% tretinoin cream and the original 0.1% cream were equivalent for improving sun-damaged skin, and 0.1% caused significantly more irritation. You can move to a higher strength later if you need it.
What strength of tretinoin should I start with over 40?
Start with 0.025% cream. In that randomized, double-blind trial, people used 0.1% or 0.025% cream once a day for 48 weeks. Both strengths beat placebo for sun-damaged skin, and they were equivalent to each other. The difference was irritation: 0.1% caused significantly more. You get the same improvement with less redness and peeling, so you're more likely to keep going for the months the results take.
The 0.1% strength was the one first used for sun-damaged skin, and it proved too irritating for many people. That's why the lower strength was tested. A 2024 systematic review of strengths from 0.02% to 0.1% found the same thing across studies: redness and irritation came up more often at higher strengths.
You can move up later. In one small open study of 32 people with sun-damaged faces, a very high 0.25% cream was tolerated when the strength was raised gradually and people used moisturizer, and their skin showed early improvement. Still, the 48-week trial found 0.025% equivalent to 0.1%. If your skin is improving on the low strength, you can stay there.
Is tretinoin cream or gel better for skin over 40?
Strength matters more than cream vs gel. In two 21-day irritation studies in healthy adults, the low-strength creams and gels landed in the same middle group, and the 0.1% cream was the most irritating every time.
| Formula | Strengths sold | Base | Where it landed in 21-day irritation tests |
|---|---|---|---|
| Cream (Retin-A) | 0.025%, 0.05%, 0.1% | Water-based cream | 0.025%: middle group in both studies. 0.05%: most irritating group in one study, middle group in the other. 0.1%: most irritating group in both studies. |
| Gel (Retin-A) | 0.01%, 0.025% | Gel that is 90% alcohol | 0.01% and 0.025%: middle group, alongside 0.025% cream |
| Microsphere gel (Retin-A Micro) | 0.04%, 0.06%, 0.08%, 0.1% | Gel, applied as a thin layer once daily in the evening | 0.1%: middle group, alongside 0.025% and 0.05% cream. Other strengths not tested. |
Both labels are written for acne. The Retin-A label says long-term safety and effectiveness for other conditions haven't been established. The results for wrinkles and sun damage on this page come from the photoaging trials, not from the labels.
How do I start tretinoin without wrecking my skin?
- Keep the routine plain. Wash gently with a mild, non-medicated cleanser, rinse, pat dry, then apply a thin layer once in the evening.
- Begin with a few nights a week. In a small trial of postmenopausal women, 0.05% tretinoin cream used three times a week on the forearms significantly lowered sun-damage scores over 24 weeks. If your skin is getting irritated, ask your clinical team about spacing your nights further apart.
- Expect some reaction at first. The label lists dryness, redness, swelling, peeling, itching, blistering, burning or stinging. To cut irritation, wash gently, don't scrub and use a moisturizer.
- Check your other products. Tell your clinical team if you use anything with salicylic acid, benzoyl peroxide, sulfur or resorcinol, because the label says these can add to the irritation.
- Step up your sun protection. Wear SPF 15 or higher and protective clothing when you're in the sun, and skip tanning beds and sunlamps. One commenter on r/30PlusSkinCare put it plainly: "Retinoids like tret makes you MORE sun sensitive. Your first step is upping your sun protection."
- Skip damaged skin and harsh weather. Don't put it on eczema patches or on sunburned skin until it heals, and use less or none in severe wind or cold.
- Mention pregnancy or breastfeeding. Tell your clinical team before you start if you're pregnant, planning a pregnancy or breastfeeding. The label says it isn't known whether it can harm an unborn baby.
- Know that very little reaches the rest of your body. Only about 1 to 2% is absorbed through the skin. In the studies reviewed, side effects were mostly mild to moderate skin reactions, such as more sun sensitivity, and none were severe.
How long does tretinoin take to work on wrinkles, and what won't it fix?
Tretinoin was already working by week 12 in a split-face trial. In that trial of people with moderate to severe sun damage, every strength tested, including 0.025%, significantly improved overall sun damage, fine lines, deeper wrinkles, brightness, blotchy pigment and rough texture.
Plan on months, not weeks. The strength comparison that supports starting low ran for 48 weeks. A strength you can use most nights for that long will do more for you than a stronger tube you abandon in week three.
Tretinoin is the treatment mentioned most in recent Reddit threads about wrinkles and crepey skin. It came up in 38 of 70 of those threads, more than Botox (26).
It's studied for lines, tone, spots and texture, not for sagging or lost volume. A 37-year-old who had used "Tretinoin for about 10 years" wrote that "my skin feels like it has lost firmness almost overnight." The top reply, with 1,741 upvotes, said: "no amount of creams and “drink more water” suggestions will stop volume loss." If smoother lines, more even tone and fewer sun spots are what you want, tretinoin is aimed at those changes.
Our clinical team can prescribe tretinoin if it fits your goal. Tell them how your skin reacted in the first weeks so they can decide on your next step, whether that's staying put or moving up.
Take the quiz so your clinical team can see whether tretinoin fits your goal of smoother lines, more even tone and fewer sun spots. They can choose a starting strength your skin can live with for the months it takes to see results.

Is 0.05% tretinoin too strong to start with?
For most beginners over 40, 0.025% cream is the gentler place to start. In irritation tests, the 0.05% cream landed in the most irritating group in one test and the middle group in another, while 0.025% cream stayed in the middle group both times. The 0.05% cream has still worked when used three times a week, so ask your clinical team which strength suits your skin.
Should I use retinol before tretinoin?
Retinol isn't a required first step, since starting tretinoin at 0.025% a few nights a week is already a gentle way in. In one small study, a retinol cream improved sun damage about as much as a low-strength tretinoin and caused less redness. A review of several studies found tretinoin worked better than retinol overall, with more irritation at higher strengths.
Can I put tretinoin around my eyes?
Keep tretinoin out of your eyes, because the label says to keep it away from the eyes, mouth, corners of the nose and other moist areas. Wrinkles around the eyes did improve in a study of women with sun-damaged faces who used tretinoin cream. Ask your clinical team how close to your eyes you should apply it.
What is the difference between Retin-A and Retin-A Micro?
Retin-A comes as a cream in 0.025%, 0.05% and 0.1% and as an alcohol-based gel in 0.01% and 0.025%, while Retin-A Micro is a microsphere gel in 0.04%, 0.06%, 0.08% and 0.1%. Retin-A Micro is applied as a thin layer once in the evening. In irritation tests, Retin-A Micro 0.1% was gentler than Retin-A 0.1% cream and landed alongside the lower-strength creams. Both labels are written for acne, and the wrinkle and sun damage results come from separate skin aging studies.
