Fat freezing results are measured in millimeters. In the review, one study found 2.8 mm less inner-thigh fat at 16 weeks, and another found 2 mm less fat under the chin at 12 weeks. Between 73% and 93% of people were satisfied across the fat freezing studies.

Emsculpt Neo adds muscle to the picture. In a multicenter MRI study of the flanks, 73 people enrolled. At three months the treated area showed 30.5% less fat and 29.0% more muscle, while their BMI stayed the same. In that study, 77.3% were satisfied and no one had a serious side effect. The study had no comparison group, so you can't tell how much of the change a placebo would have matched.

The studies measured results in different ways, so none of these devices can be called the best. Most need several sessions over weeks or months, and the review calls for more trials on how well results and safety hold up long term.

Will non-surgical fat reduction help me lose weight?

No. CoolSculpting is not a weight loss procedure. It treats a stubborn area that diet and exercise haven't shifted, and it isn't designed to change your weight much.

It is made for women near their target weight. CoolSculpting is FDA-cleared for a BMI of 30 or less for most body areas, and up to 46.2 for under the chin.

Here is the quick test. If the fat you mind is one pocket, such as your flanks, lower belly or inner thighs, and your weight is where you want it, a device fits the job. If you would also like your jeans to fit differently all over, a device won't get you there.

Some women notice a change within a month, but final results can take several months, and more than one session may be needed. If you don't gain weight, the results can be permanent.

What is the best non-surgical treatment for a double chin?

Kybella (deoxycholic acid) is the FDA-approved injection for moderate to severe fullness under the chin in adults. It isn't approved for fat anywhere else, and the label recommends against using it elsewhere.

The trials looked a lot like you. In two placebo-controlled trials, the average age was 49 and 85% of participants were women. Twelve weeks after the last treatment, 70.0% and 66.5% of Kybella patients improved on both their own rating and their clinician's rating, compared with 18.6% and 22.2% on placebo.

On MRI, 43% of Kybella patients lost at least 10% of their chin fat, compared with 5% on placebo. You can have up to 6 treatments, spaced no less than 1 month apart, at $1,200 to $2,000 per session.

The risk to know about is nerve injury. In the trials, some people developed an uneven smile or facial weakness from injury to a nerve along the jaw. When Kybella has been injected outside the chin, people have reported vision problems, nerve injury and lumps that may not go away. Choose a clinician who will inject it under the chin only.

What are the risks of CoolSculpting?

Most side effects are mild and pass: swelling, numbness and temporary redness. The rare serious one is paradoxical adipose hyperplasia (PAH), where the treated fat grows larger and firmer instead of shrinking. It happens in less than 1% of patients and is three times more common in men than in women.

PAH has been estimated at about 0.05% to 0.4% of treatments.

When PAH happens, the treated fat shrinks at first and then grows past its original size, and fixing it usually means surgery such as liposuction. Fat freezing is also offered in med spas and offices that aren't necessarily overseen by a doctor, where staff may not recognize PAH.

Before you book, ask the clinic how many applicators and sessions your area needs, the total price to finish treatment and the credentials of the person treating you.

What if I have more than one area to lose?

Treating your belly, flanks and thighs one area at a time adds up. At $2,000 to $4,500 per area, several areas or repeat rounds can push CoolSculpting to $6,000 to $12,000 or more, and none of it changes your weight.

One 38-year-old woman posted a year on tirzepatide (Zepbound) in r/Zepbound, in a thread titled "One year difference - 264 to 190." She wrote: "Been on zep for a year… worked my way up to 12.5 slowly." She went from 264 to 190 pounds, and added: "Weight loss slowed to about 4-6 lbs a month for the last 5 or so months, but I upped my strength training at that time as well."

Side effects don't suit everyone. In another r/Zepbound thread, "Throwing in the towel," one poster wrote: "I started at the lowest dose (2.5 mg), and never increased, due to side effects." They lost 23 pounds and stopped after about two months, with this advice: "Listen to your body while it whispers, before it starts to yell." Ask your clinical team what to watch for.
