Quick answer

The best online spironolactone provider for hair loss or acne prescribes the oral generic tablet, asks about your other medicines and pregnancy plans, checks your potassium if you are over 45, and follows you for at least six months. Spironolactone is a cheap generic pill, and using it for hair loss or acne is off-label, so most of what an online plan charges should pay for that screening and follow-up. Spironolactone can help thinning hair when added to minoxidil and can improve adult acne, but give it about six months before you judge it.

Key takeaways

  • A good online spironolactone provider asks about pregnancy, birth control and everything else you take, because some blood pressure drugs, painkillers and potassium supplements can push your potassium too high.
  • Women who added a spironolactone tablet to minoxidil were more likely to see clearly fuller hair after six months than women on minoxidil alone, though irregular periods were common.
  • Spironolactone improved adult acne more than a dummy pill by six months, but the difference was hard to see at three months, so judge it at six.
  • If you are over 45, expect regular potassium blood tests, because high potassium becomes more common with age.
  • The generic pill is cheap (a 30-day supply of 100 mg cost $14.95 at Walmart in 2019), so a plan that charges well above that should include your blood work and regular check-ins.

The best online spironolactone provider for hair loss or acne is one that prescribes the oral generic tablet, screens your other medicines and pregnancy plans, checks your potassium if you are over 45, and follows you for at least six months. No one has compared online providers head to head, so that checklist is your best test. Spironolactone is a cheap generic pill, so most of what an online plan charges pays for the screening and follow-up. Using it for hair loss or acne is off-label: its FDA label covers heart failure, high blood pressure, swelling and an adrenal hormone condition.

What should an online spironolactone provider check before prescribing?

  • Whether off-label use makes sense for you. The Aldactone label covers heart failure, high blood pressure, swelling (edema) and primary hyperaldosteronism, an adrenal condition that makes too much of the hormone aldosterone. Hair loss and acne are not on that list, so the provider's judgment matters more than usual.
  • Whether you are pregnant or trying. The label says to avoid spironolactone in pregnant women, or to tell them about the possible risk to a male baby.
  • Your birth control. In the largest acne trial, women got contraception advice at every visit and a pregnancy test at the start. A provider should cover the same ground.
  • Your full medicine list. The label warns that ACE inhibitors, ARBs, NSAID painkillers, heparin and trimethoprim can raise potassium. It also tells patients to avoid potassium supplements, salt substitutes and high-potassium foods. If nobody asks what else you take, that is a gap.
  • Whether you are getting the tablet or a topical blend. Some online "spironolactone" hair products go on your scalp. Curology's Hair Formula Rx, for example, is a once-a-day scalp solution, and spironolactone is one of six possible ingredients its providers can pick. The hair trial in the next section used the tablet.
  • A check-in about your periods. Irregular periods are the most common side effect, affecting 15% to 30% of women according to a review of spironolactone side effects. They become more likely at higher doses, and the pill or an IUD can ease them.

How well does spironolactone work for hair loss and acne?

Who was studied Dose How long What changed Most notable side effect
Hair loss (trial) 48 premenopausal women aged 21 to 45. Every woman used minoxidil 3% twice a day 100 mg tablet, or a placebo 24 weeks On photos, 38% on spironolactone had moderate to marked improvement, against 9% on placebo Irregular periods in 37.5% on spironolactone, significantly more than on placebo
Acne (SAFA trial) 410 women with persistent facial acne 50 mg to start, with almost all moving up to 100 mg 24 weeks 82% on spironolactone said their acne had improved, against 63% on placebo Headaches in 20%, against 12% on placebo. No serious side effects

Hair. In the hair trial, the spironolactone group also gained more thick hairs per square centimeter, but that difference was not statistically significant. Women in both groups grew denser, thicker hair, so the minoxidil-only group improved too. The authors called spironolactone generally well tolerated.

The hair evidence is still small. One review pooled 413 patients from mixed studies. It noted that earlier studies found better results at 12 months than at 6, so a good provider plans for at least a year before calling it.

Women's experiences vary. One woman wrote, "My hair shedding decreased so much after a month." Another replied, "I was on 100mg for almost 4 months and didn't notice a single difference."

Acne. At 12 weeks the acne trial showed no significant difference (72% improved on spironolactone against 68% on placebo). The gap opened by 24 weeks. Judge it at six months, not three.

Ask any provider what happens when you stop. One woman wrote that "spiro helped it clear but gave me side effects and when i came off the med, the acne came back 10x worse than before."

Every woman in that hair trial used minoxidil, and our clinical team can prescribe minoxidil if it fits your hair goal. For skin, they can prescribe tretinoin if it fits. If you take spironolactone or plan to, tell your clinical team so your plan accounts for it.

Do I need blood tests on spironolactone if I'm over 45?

Yes, the researchers recommend them. The "no labs needed" advice comes from studies of younger women. In healthy women aged 18 to 45 taking spironolactone for acne, only 13 of 1,802 potassium tests (0.72%) came back high, according to the side effects review. Those numbers say nothing about older women.

One small study found a different pattern in older women. High potassium turned up in 1 of 112 women aged 18 to 45, and in 2 of 12 women aged 46 to 65. Kidney function declines with age, so the side effects review's authors recommend regular potassium checks for women over 45.

For spironolactone's approved uses, the label says to check potassium within 1 week of starting or raising the dose, and regularly after that. Checks may need to be more frequent if you take other drugs that raise potassium or if your kidneys work less well.

If you are over 45 and a provider skips the blood test, that provider is a poor fit for you. If you are under 45 with no kidney problems and no potassium-raising medicines, ask your clinical team whether you need a test.

How much should spironolactone cost online?

The pill itself is cheap. In 2019, a 30-day supply of 100 mg cost $14.95 at Walmart. A potassium test cost $5.11, plus $18 for the blood draw, at Medicare rates (side effects review).

So most of what you pay an online provider covers the visit, the subscription and the follow-up. Compare a plan's monthly price with the pill's pharmacy cost, and check whether labs and check-ins are included. A plan that charges well above the pill's cost should cover your blood work and regular check-ins.

Curology's Hair Formula Rx comes as a 60-day supply and requires a subscription. After you take its hair quiz, a provider picks three active ingredients. Check which ingredients you would get before you compare it with an oral spironolactone plan.

If you want thicker hair or clearer skin, take the quiz. Our clinical team can build a plan around minoxidil for your hair or tretinoin for your skin if either fits. Tell them about any spironolactone you take or are considering.

FAQ

Can you take spironolactone and minoxidil together for hair loss?

Yes, and the hair results for spironolactone come from women using both. Every woman in that research used minoxidil liquid twice a day, and those who added a 100 mg spironolactone tablet were more likely to see clear improvement after six months than those on minoxidil alone. The women on minoxidil alone improved too, and irregular periods were the most common downside of adding the pill. Our clinical team can prescribe minoxidil if it fits your hair goal, so tell them about any spironolactone you take or are considering.

What dose of spironolactone do women take for hair loss?

In the hair research in this article, women took a 100 mg tablet once a day for six months. In the acne research, women started at 50 mg and almost all moved up to 100 mg. Irregular periods become more likely at higher doses, and earlier hair studies found better results at a year than at six months, so a good provider plans for at least a year before deciding whether it worked.

Does acne come back after stopping spironolactone?

Some women say it does, but the research in this article did not measure what happens after stopping. One woman wrote that spironolactone cleared her acne but gave her side effects, and when she came off it the acne came back worse than before. That is one woman's account, not a measured result, so ask any provider what happens when you stop before you start.

Is spironolactone safe to take after menopause?

The research here does not answer that directly, because the hair research included only premenopausal women aged 21 to 45. What it does show is that high potassium turns up more often in older women, since kidney function declines with age, so the researchers recommend regular potassium blood tests for women over 45. If you are past menopause and a provider skips the blood test, that provider is a poor fit for you.
