Across randomized trials, it smoothed fine and coarse wrinkles and evened out blotchy pigment and dullness compared with the same cream without the drug (tretinoin meta-analysis; review of tretinoin trials). What is making my skin texture rough and uneven?

Rough texture usually has one of two causes. Sun and age leave skin grainy, dull, blotchy and finely lined. Acne scarring leaves small pits and dents. Each needs a different treatment.

One 37-year-old woman described the sun-and-age kind in a thread on r/30PlusSkinCare: "I feel over the last couple years my skin texture has changed and become grainy and I have a lot of wrinkles on my eyes now."

Much of that grainy, uneven texture comes from sun damage. Unlike the aging your genes set, sun damage can be prevented by limiting sun and wearing sunscreen regularly (tretinoin meta-analysis). Sunscreen belongs in every plan on this page.

Crepey or loose skin on your body after pregnancy or weight loss is a different problem. The tretinoin trials below tested facial skin, so this page is about your face.

Does tretinoin really smooth rough skin texture?

Yes. A pooled analysis of eight randomized trials included 1,361 people, and the median age in those trials ranged from 29 to 76. Tretinoin improved both fine and coarse facial wrinkles compared with the same cream without the drug. Follow-up ran from 16 weeks to 2 years. In six of those trials, people on tretinoin also rated their own overall improvement higher than people on the plain cream. That rating covered how their skin looked and felt.

Tretinoin also evens out uneven skin. A review of seven randomized trials found the same result in every trial: tretinoin improved wrinkling, blotchy dark patches, sallowness (a dull, yellowish cast) and sun spots. Four of the trials enrolled only women, and women made up most of the people in the other three. Changes showed as early as 1 month and lasted past 24 months.

Skin biopsies show how it smooths. In five trials, tretinoin thickened the top layer of skin, packed the outermost surface layer more tightly and lowered pigment. All three changes are tied to smoother texture. One trial also found signs that skin was making more collagen (tretinoin meta-analysis).

How bad is the tretinoin peeling at the start?

Expect some irritation, mostly in the first month. In a 44-week study of 355 people, here is what happened in the first 4 weeks on tretinoin:

  • 44% had redness.
  • 35% had flaking.
  • 29% had burning or itching.

Irritation is more likely on tretinoin than on a plain cream. In the trials, though, the common reactions generally cleared with continued use or basic supportive skin care (tretinoin meta-analysis).

The peeling is not what smooths your skin. One trial found no link between how irritated skin got and how well it responded. Among people whose skin improved satisfactorily to excellently, only half rated their irritation 5 or higher on a 9-point scale. You do not need a raw face for tretinoin to work.

Retinol and retinaldehyde are the over-the-counter cousins of tretinoin. Both caused less irritation than tretinoin in tolerance testing. In long-term use, redness, flaking and burning were less frequent on retinaldehyde. That study measured comfort, not results, so it does not show they smooth texture as well. Tell your clinical team how your skin is reacting so they can adjust your plan.

Is microneedling or a chemical peel better for skin texture?

For acne scars, the combination beats either one alone. In a pooled analysis of 9 studies with 566 people, microneedling plus a chemical peel outperformed both treatments used alone. Between 40 and 50% of people on the combination improved their scars by 75% or more, compared with 13% or fewer on either treatment alone. Side effects were mild and short-lived. Microneedling left skin red and sore for a couple of days: 50 to 60% of people in the microneedling groups had pain and redness, which settled within 48 to 72 hours.

These in-office trials were done on acne scars. None of the research compared peels or microneedling directly with tretinoin for sun and age texture.
