Cellulite comes from the structure under your skin. Fibrous bands called septae tie your skin to the muscle underneath. When they tighten unevenly, they pull the skin down while fat pushes up between them. That gives you the cottage cheese look, according to the American Board of Cosmetic Surgery's guide to cellulite treatments.
That structure is why cellulite is so common. An estimated 85 to 90% of adult women have it, including very lean women, and losing weight rarely gets rid of it altogether. Aging can make it look worse as skin loses elasticity.
Creams and massage work on the surface, and the bands sit underneath. The board says even high-quality creams are rarely effective and "rarely a worthwhile investment." Massage improvements are temporary.
So the treatments worth paying for are the ones that reach the bands. That is how each option below is judged.
Which cellulite treatments have the best evidence?
The treatments that cut or dissolve the bands have the strongest evidence. QWO, the enzyme injection, had the strongest trial data, but it is no longer made. Cellfina and Avéli cut the bands, and you can still book them.

Both ways of releasing the bands look effective. A 2024 review of 14 studies and 1,254 patients found that enzyme injection and subcision (cutting the bands) both look effective for thigh and buttock cellulite. No study has compared them head to head, so neither one has been shown to work better.
QWO's maker, Endo, stopped producing and selling it in December 2022 because of excessive, unpredictable bruising and a risk of long-lasting skin discoloration, the board reported. It is still FDA-approved, and doctors can use any unexpired stock they have, but the board expects it to get harder to find a provider over time. In practice, that leaves Cellfina and Avéli as the band-release options you can book.
Insurance does not cover Avéli, so plan on paying the full price yourself.
How bad is the bruising and downtime after cellulite treatment?
Plan on bruising either way. In the 2024 review, about 99% of people bruised after subcision and about 89% after enzyme injection. Neither treatment caused serious complications.
Side effects overall were more common with the enzyme injection. It brought more pain that needed painkillers and more noticeable skin discoloration.
The cosmetic surgery board puts treatment time at 30 to 90 minutes. Most people are back at work in 1 to 3 days and fully recovered in 1 to 2 weeks. Book around beach trips and anything where your thighs are on show.
Go in expecting smoother skin, not perfect skin. The board says that whichever method you choose, you should expect it to reduce your cellulite, but not fully erase it.
Will losing weight on a GLP-1 get rid of my cellulite?
Probably not on its own. A 12-month observational study followed 92 people with moderate to severe (grade II to III) cellulite. Those on tirzepatide alone lost about 11 kg on average. Their cellulite improved by about half a grade (0.50). The groups using a topical treatment improved by 1.68 grades, and the groups using both improved by 1.73.
In that study, how much a person's cellulite improved did not track with how much weight or fat she lost. The study was not randomized, so it shows a link, not proof.
GLP-1 drugs cause fast, significant weight loss. A 2025 dermatology review notes that fast loss can also reduce skin elasticity and leave skin looser. Surgeons advise waiting for weight to settle before surgical procedures, and on a GLP-1 it is harder to predict when your weight will stabilize.
The cosmetic surgery board says cellulite treatment works best when you are happy with your current weight and not significantly overweight. So if weight is part of your goal, do it in this order. Lose the weight first, then have the bands released once your weight is stable.
