Quick answer
The cheapest cash pharmacy depends on whether your drug has a generic. Brand-name GLP-1s cost least at the makers' own pharmacies: Zepbound starts at $299 a month through LillyDirect, and Wegovy pens start at $199 a month through NovoCare Pharmacy for a new patient's first two fills (through December 31, 2026), then $349. Generic tretinoin 0.05% cream costs $28.52 at Mark Cuban Cost Plus Drug Company, generic estradiol starts around $29 a month with a free GoodRx or SingleCare coupon, and every one of these routes needs a prescription.

Key takeaways
- The cheapest place to fill a prescription with cash depends on whether your drug has a generic version, and every route still needs a prescription.
- LillyDirect and NovoCare sell Zepbound, Wegovy and Ozempic for the lowest cash prices, the Wegovy pill and Foundayo start at $149 a month, and TrumpRx.gov matches those starting prices without beating them.
- Your GLP-1 cash price rises as your dose goes up, and at Zepbound 10, 12.5 or 15 mg a refill completed later than 45 days after your last delivery raises the price from $449 to $699 a month.
- A 20 g tube of generic tretinoin 0.05% cream costs $28.52 plus $5 shipping at Mark Cuban Cost Plus Drug Company, and generic estradiol gel or vaginal cream costs about $29 to $50 a month with a GoodRx or SingleCare coupon.
- Commercial insurance with a maker's savings card can bring a GLP-1 copay to as little as $25 a month, but copay cards exclude Medicare and Medicaid, and cash payments generally do not count toward your deductible.
The cheapest place to fill a prescription with cash depends on one question: does your drug have a generic version? Brand-name GLP-1s such as Zepbound and Wegovy have set cash prices through the makers' own pharmacies. Injections start at $299 a month through LillyDirect and $199 a month through NovoCare Pharmacy. Generic tretinoin cream costs $28.52 at Mark Cuban Cost Plus Drug Company, and generic estradiol starts around $29 a month with a free GoodRx or SingleCare coupon. Every one of these routes still needs a prescription.
For brand GLP-1s, a coupon hunt won't save you much. The savings come from your dose and from refilling on time. Miss one refill window and your monthly price can jump by $250.
Where do you get the lowest cash price on Zepbound or Wegovy?
The makers' own pharmacies sell them for the least: LillyDirect for Zepbound and NovoCare for Wegovy and Ozempic. Here is what each dose costs when you pay cash.

The pills start lower than the injections, at $149 a month for the lowest doses of both the Wegovy pill and Foundayo.
You may have seen TrumpRx.gov. It is a free federal price list. It does not sell or dispense drugs, and its discounts apply only to cash-paying patients. Its starting prices are Wegovy pill from $149, Wegovy pen from $199 and Zepbound from $299. Those match the makers' own starting prices, so it won't beat LillyDirect or NovoCare.
Budget for supplies with Zepbound. The vial needs syringes and needles, and the KwikPen needs pen needles. Both are bought separately.
Hot or Nothing offers semaglutide (the drug in Wegovy and Ozempic) and tirzepatide (the drug in Zepbound). If you want your clothes to fit again, our clinical team can prescribe one when it fits your goal.
Why does my GLP-1 cash price go up as my dose goes up?
Most women start on the lowest dose and move up over months, and each step up can raise the cash price. Zepbound goes from $299 a month at 2.5 mg to $399 at 5 mg.
One r/Zepbound poster climbed that ladder over a year: "Been on zep for a year… worked my way up to 12.5 slowly and hopefully this is where I'll stay." Her post is titled "One year difference - 264 to 190."
At 7.5 mg and above, a late refill costs real money. Zepbound at 10, 12.5 or 15 mg costs $449 a month only if each refill is completed within 45 days of your last delivery. If it isn't, you pay $699. That is $250 more a month for the same medicine. At 7.5 mg, a late refill takes you from $449 to $499. Set a reminder.
Some women stay on a lower dose longer, and that keeps the monthly price lower while they're there. Another r/Zepbound poster wrote: "Finally upped my dose of 2.5 mg (16 weeks) to 5 mg." Your clinical team sets your dose with you, based on how you're doing, not on the price list.
The $199 Wegovy and Ozempic starter price covers only the first two monthly fills of 0.25 mg or 0.5 mg for new patients, and the offer ends December 31, 2026. After that, plan for $349 a month.
Where is generic tretinoin cheapest without insurance?
Start with Mark Cuban Cost Plus Drug Company. A 20 g tube of tretinoin 0.05% cream costs $28.52 there, plus $5 standard shipping and any taxes. The site lists the retail price at other pharmacies as $80.90.
Cost Plus shows you where the money goes. That $28.52 is $20.45 for manufacturing, a 15% markup of $3.07 and $5 for pharmacy labor. The $5 shipping comes on top of that.
Cost Plus prices its generics at cost plus a 15% markup and already carried almost 1,000 medications by late 2022. That makes it a good first place to check the price of any generic skin or hormone prescription.
You still need a prescription. Cost Plus sells tretinoin as a cream or gel, in 0.025% or 0.05% strength, and in 20 g or 45 g tubes, so your price depends on what your clinical team writes.
If clearer skin is the goal, Hot or Nothing offers tretinoin, and our clinical team can prescribe it if it fits your skin.
How much should estradiol cost with GoodRx or SingleCare?
Generic estradiol gel or vaginal cream costs about $29 to $50 a month at a retail pharmacy with a free GoodRx or SingleCare coupon. That works out to about $348 to $600 a year.
Brand-name versions can cost far more. Brand Estrace vaginal cream costs $452.52 a tube with SingleCare. The generic estradiol vaginal cream in the same 42.5 g tube costs as little as $32.51, about 93% less.
If your gel has no generic, use the coupon and check prices at several pharmacies. EstroGel has no US generic. With SingleCare, the same 50 g bottle cost $74.03 at CVS, $143.50 at Walmart and $143.54 at Walgreens.
Elestrin has no generic either. It costs about $158 a month at the average cash price (about $317 for a two-bottle package), or about $94 a month with a GoodRx coupon (about $188 for the package).
Hot or Nothing offers bioidentical estrogen too. Our clinical team can prescribe it if it fits your goal.
Should I use insurance, a copay card or pay cash?
Your coverage decides the cheapest route.
| Your situation | Cheapest route | What to know |
|---|---|---|
| Commercial insurance that covers the drug | The maker's savings card with your insurance | A savings card can bring your copay to as little as $25 a month. The Wegovy offer caps savings at $100 a month. The Zepbound pen card caps savings at $100 a month and $1,300 a year. |
| Medicare or Medicaid | Not a copay card | Copay cards exclude people in government health programs. LillyDirect points eligible Medicare Part D enrollees prescribed Zepbound KwikPen or Foundayo for weight management to the Medicare GLP-1 Bridge program. |
| No coverage | Cash through LillyDirect or NovoCare for a brand GLP-1, and Cost Plus or a GoodRx or SingleCare coupon for generics | Under the Lilly terms and the TrumpRx coupon terms, self-pay generally does not count toward your deductible or out-of-pocket maximum. NovoCare self-pay requires you to agree not to submit a claim to any insurer. |
Lilly and Novo Nordisk both say they can change or end these offers at any time, so check the current price before each refill.
Every cash price on this page starts with a prescription. Take the quiz to start a plan with our clinical team for your weight, skin or hormone goal, then compare where to fill it.

Does paying cash for Wegovy count toward my deductible?
Paying cash for Wegovy generally does not count toward your deductible. NovoCare's self-pay terms require you to agree not to submit a claim to any insurer for the medicine. The TrumpRx coupon terms also say self-pay generally does not count toward your deductible or out-of-pocket maximum.
Can I use a Zepbound savings card if I have Medicare?
No, Zepbound copay savings cards exclude people in government health programs such as Medicare and Medicaid. The KwikPen self-pay savings card also requires that you not seek payment back from any insurer, including government programs. LillyDirect points eligible Medicare Part D enrollees prescribed Zepbound KwikPen for weight management to the Medicare GLP-1 Bridge program.
Is TrumpRx cheaper than buying from LillyDirect?
No, TrumpRx.gov lists Zepbound starting at $299 a month, the same starting price LillyDirect charges. TrumpRx is a free federal price list that does not sell or dispense drugs, and its discounts apply only to cash-paying patients. For some drugs, the listed price is available only through the maker's own website or a few mail-order pharmacies.
Do I need a prescription to order from Cost Plus Drugs?
Yes, Mark Cuban Cost Plus Drug Company lists tretinoin as prescription required. You create an account, and Cost Plus emails you once it receives your prescription from your prescriber. Your price depends on the form, strength and tube size written on that prescription.
